FINANCIAL PROJECT ID 432287-1-58-01 FINANCIAL PROJECT ID 432287-1-58-02 Scope of Work / Standard Form Contract (Incorporated by Reference and revised as indicated within the solicitation) Dawson Road Over Pritchett Mill Bridge Number 484046 Bridge Replacement Project. Scope of Work: The safety improvements under this contract consist of the replacement of the Dawson Road over Pritchett Mill Creek Bridge, existing Bridge Number 484046. The project is ocated in northern Escambia County between US 29 and the Escambia River, a few miles south of Century. The existing functionally obsolete single span bridge carries two lanes of traffic. The proposed 76'-0" long x 36'-8" wide replacement bridge will also carry two lanes of traffic. The bridge will be a three-span continuous flat slab concrete structure. Associated roadway improvements on Dawson Road include approximately 270 feet of roadway replacement, curb and gutter, guardrail improvements, and grading for ditches. The project will require utility coordination, working near existing utilities and the relocation of a 3-inch water line and providing conduit for a 3/4-inch gas main to be relocated by others. Work will be performed under a General Permit from the Florida Department of Environmental Protection (FDEP) and Nationwide Permit (NWP) from the Army Corps of Engineers. Traffic control will include closing Dawson Road for the duration of the project. The work shall be substantially completed within one hundred forty (140) calendar days from the Commencement Date. The Bidder agrees to fully complete all work included above within thirty (30) consecutive calendar days from the date of Substantial Completion. Liquidated Damages of $1,694.00 will be assessed for each calendar day that completion of the project is delayed. All work to be accomplished under this bid shall be the responsibility of the Bidder, and failure of subcontractors to perform shall not relieve the Bidder of any Liquidated Damages. A Bid Bond in the amount of five percent (5%) of the total base bid is shall be furnished by each bidder. Bidder further acknowledges that all the work outlined above may not be required at the discretion of Escambia County. The total will be subject to total funds available during the work. Addendum Posting December 3, 2021 The County shall require the successful Offeror to furnish performance and payment bonds, under pledge of adequate surety and covering up to one hundred percent (100%) of the dollar value of the award on the forms provided by the County. Such bonds shall be issued by sureties authorized to act as a surety by the State of Florida. Bonds of the successful Offeror shall be reviewed by the Office of Purchasing to assure compliance, then recorded at the Office of the Clerk of the Circuit Court Recording Office, 223 Palafox Place, 1 st Floor, Pensacola, Florida 32502, by the successful Offeror at their expense before the contract is executed by the County. The cost of recording is $10.00 for the first page and $8.50 for each additional page.
