Proposals must be sealed and submitted on the proposal forms and returned clearly marked 'City of Lake Geneva 2022 Joint and Crack Filling' with name of proposer, date and time of opening. No undated, unsigned, late, incomplete, emailed or faxed proposals will be considered. The City is not responsible for late or misdirected mail or cartage therefore hand delivery is advised. Bidders must submit pre-qualification packet prior to the opening of bids by contacting the City Clerk's office or visiting the City website. Bid will not be awarded until Pre-Qual is approved. All bids shall be F.O.B. destination, prepaid and allowed to the Clerk's Office with no charges for packing or cartage. The City reserves the right to reject any or all proposals and to accept any proposal deemed most advantageous to the City of Lake Geneva. The City of Lake Geneva is exempt from Federal Excise Tax and State Sales Tax; therefore, proposals should be made exclusive of these taxes. A Tax Exemption Certificate and/or Tax Exemption Registry number will be furnished to the successful proposer. Insurance Requirements are: $1,000,000 General Liability, Worker's Compensation - Statutory Limits, $5,000,000 Employer's Liability. See pre-qualification form on website or Clerks Office. Successful bidder shall properly hold the City of Lake Geneva harmless from all damages occurring in any way by bidders acts or negligence, or that of his employees, agents or workers. Completed Insurance questionnaire must be submitted with your proposal. Contractor to include any clarifications to the specification in his proposal. Proposed price shall be all-inclusive for Joint & Crack Cleaning & Sealing with no part being 'to be done by others.
