Everetts Production Well Abandonment The project will involve several activities including the following:
1. Abandon one 14-inch diameter production well with a depth totaling 993 feet.
2. Abandon the one well following Virginia Administrative Code 12VAC5-630-
450.
3. A licensed well driller in Virginia will complete Virginia Department of
Environmental Quality Well Abandonment Form GW-5 for Everetts well,
executing the form and provided a license number.
4. Demolish the well house and perimeter fence surrounding the abandoned well
and remove the building materials.
5. Removal of wooden pole and equipment where electrical power feeds the well
(onsite pole, not the Dominion pole).
6. Restore the well site to the original condition at the start of the project.
B. The Contractor shall check the access to each well and plan the operation
accordingly. The location of Everetts well, particularly appears remote from a water
source, and may require hauling water to the site to complete development.
C. The project is located on the Izaak Walton property, located at 480 Kings Fork Road,
Suffolk, VA 23434. The Contractor shall access the Izaak Walton property behind
Kings Fork Middle School between the hours of 8:30 am and 1:00 pm or after 3:30
pm Monday through Friday, or any time on Saturday or Sunday. This will ensure that
vehicles are not in the bus ramp during arrival and dismissal times.
D. Grassy areas shall be matted for vehicular traffic, and the Contractor will repair any
damage to the grounds or asphalt that occurs while accessing the site. Contractor will
obtain approval for any tree and limb clearing if necessary to access the site from the
City of Suffolk Department of Public Utilities. The Contractor shall furnish all labor, materials, equipment and means to construct the
project entitled Abandoning Everetts Production Well for the City of Suffolk
Department of Public Utilities as shown on Drawings and described herein. The work
includes, but is not limited to, the following:
1. Abandoning production well and any associated aboveground structures at the
City of Suffolk Everetts well.
2. Certifying proper abandonment on Well Abandonment Form, GW-05.
3. Restore the areas around each monitoring well to their original condition.
B. The above general outline of principal features does not in any way limit the
responsibility of the Contractor to perform all work and furnish the required materials,
equipment, labor and means as shown or required by the Contract Documents.
C. Materials, equipment, labor, etc., obviously a part of the work and necessary for the
proper operation and installation of same, although not specifically indicated in the
Contract Documents, shall be provided as if called for in detail without additional cost
to the Owner.
D. Successfully completing Items 1 through 3 as listed above represents the requirements
for Substantial Completion.
