LEIN Check (Department of Corrections ONLY) All contractor / vendor representatives attending Preproposal Walk Through Meeting must submit a Vendor / Contractor LEIN Request form five business days prior to the meeting date (See the attached Vendor/Contractor LEIN Request Form). The email "Subject" must include (facility name, project name, date, and time of Pre-Proposal Walk Through Meeting). COVID 19 Precautions: 1. Any individual inside a state building must wear a face mask, irrespective of vaccination status. This applies in all State owned and leased facilities throughout the State of Michigan, including all common areas, such as areas where individuals are waiting to enter a facility, cubicles, offices, conference rooms, elevators, stairwells, kitchens, bathrooms, and hallways. 2. PROJECT DESCRIPTION/SERVICES REQUESTED Based on selected recommendations from the 2018 Facilities Design Study, provide Phase 400 Preliminary Design, Phase 500 Final Design and Phase 600 and 700 services for Construction Administration.
