All bids must include: General bid pricing form Non-collusion form Tax compliance certificate Certificate of corporate vote (in the case of corporations) Certificate ofLiability Insurance and Workers Compensation Insurance Bid deposit of 5% ofthe total bid in the form of cash, money order, company check, or bid bond payable to the Town ofChesterfield Payment Bond of 50% (fifty percent) must be presented within 10 days ofthe award ofthe contract and shall be in a form satisfactory to the Awarding Authority and with a surety company qualified to do business in the Commonwealth ofMA. Proof of OSHA Training Certification is required for all employees to be employed at this worksite. At least three recent references.
