Description of Work: The proposed work consists of constructing concrete sidewalk at various park locations in Chatham, oficially known as the VILLAGE OF CHATHAM, CHATHAM PARKS, SIDEWALK IMPROVE- MENTS - CHATHAM PARKS, Chatham, Illinois. The project shall consist of excavating, forming, and placing concrete sidewalks at various locations within Chatham Community Park, Center Park-VFW and Westside Park. All work shall be in accordance with the contract documents and details and shall be performed in accordance with the Illinois Department of Transportation Standard Speciications for the work and materials speciied. All labor and materials are included as part of this Request for Proposals and shall be subject to the Illinois Department of Labor Prevailing Wages for such work. The following locations include the speciied items: Chatham Community Park 1. 589 Linear Feet, 5' wide sidewalk with detectable warnings, 4" thickness 2. A 12' x 12' concrete pad for bleachers, 6" thickness 3. Seeding and mulch restoration Center Park- VFW Park 1. 290 Linear Feet of 6' wide sidewalk with detectable warnings, 4' thickness 2. Seeding and mulch restoration Westside Park 1. 345 linear feet of 5' wide sidewalk with detectable warnings. 4" thickness 2. Two- 6' x 6'concrete pads, 4" thickness 3. Seeding and mulch restoration Alternate Bid 1. 657 linear feet of 5' wide sidewalk with detectable warnings, 4" thickness 2. 6' x 6' concrete pad,4" thickness 3. Seeding and mulch restoration 3. Method of Payment: Square footage of sidewalk installed, following a contractor-supplied schedule of values for the work to be completed.
