Site work for a water / sewer project in Pahokee, Florida. Completed plans call for site work for a water / sewer project.
Penalty damages will be charged at $250.00 per day for all calendar days that extend past the allotted time. Contract time is 60 calendar days (30 days of which include mobilization) from contract signing to substantial completion. A. Project scope of work consists of: All labor and material for the work. Work consists of the labor and material for the installation of four 6" DDCV complete systems, concrete slabs associated with each, and enclosure cages for each. Two of these systems are in the unincorporated Palm Beach County, and two of these systems are in the City of Pahokee. Each system will require the appropriate permits from each municipality. B. Full on-site supervision of a State of Florida Underground Utility and Excavation Contractor or his owner approved site supervisor is required on this project. Architect and Housing Authority must be able to be in touch with, at all times, the Contractor and/or its Field Superintendent. All responsive bids shall contain the one original hard copy, one extra hard copy, and one digital copy all with the Contractor's Name and Project Title on the cover of the sealed package. The sealed package to contain the following executed and original documents: 1. The completed Bid Proposal, section 00200 2. HUD FORM - 5369-A, Section 00205. 3. A 10% Bid Bond or certified check, Section 00210. 4. List and identify all sub-contractors, Section 00218. 5. Notarized Non-Collusion Affidavit, Section 00220. 6. Executed Disclosure of Lobbing Activities, Section 00225. 7. Notarized Anti-Kickback Agreement, Section 00230. 8. Executed Certification for Contracts, etc., Section 00235. 9. Notarized Statement of Contractor's Qualifications, Section 00240. 10. Executed Contractor's Responsibility Job Experience listing, Section 00242-3. 11. A certificate of Insurance showing Florida Worker's Compensation insurance is in place (not a WC exemption), Section 00245. 12. Executed Certificate of Non-segregated Facilities, Section 00250. 13. Certificate of non-disbarment, Section 00255. 14. Public Entity Crimes Statement, Section 00258. 15. Contractor's Section 3 Work Plan and Statement of abiding by such, Section 00260. 16. Executed Certification of Labor Standards, Section 00270. 17. Execution of Previous Participation Certification, Section 00280. 18. Photocopy of the current Contractor's State License. 19. Your SAM's number or a copy of your pending online registration for a SAM's number. All wage rates on this project shall not be less than the prevailing wage rates as determined by the U.S. Department of Labor. Certified payrolls will be a required to be submitted weekly. The Contractor must take affirmative action to ensure that all workers are employed, and that employees are treated during employment without regard to their race, color, religion, sex, national origin, age, and all other protected classes of individuals. All successful bidders are required to comply with Section 3 of the Housing and Urban Development Act of 1968, amended 1969, an amended by Section 118 of Title 1, community Development and Housing Act of 1974, as well as Executive Order 11478. Section 3 requires that to the greatest extent feasible, opportunities, training and employment be given to lower income resident of the project area, and contracts for work in connection with the project area be awarded to business concerns which are located in, or owned in substantial part of persons residing in the area of the project. Contracts will be awarded to the low bid Contractor who is proven to be responsive and responsible based on the qualifications for the project. Contractor must be registered on the "System for Award Management" (SAM) webpage and show an ACTIVE classification for this type of work. Contract will be awarded provided the bid is reasonable, and in the best interest of the Pahokee Housing Authority, and the bidder has complied with all applicable laws and the requirements of the bid documents. The Project Award is contingent upon the approval of the HUD authorities having jurisdiction and their release of the project Funds. The Pahokee Housing Authority reserves the right to reject any and all bids and to waive any informality in bids wherever it is in the best interests of the Pahokee Housing Authority.
