Renovation of an elderly care / assisted living facility in Binghamton, New York. Completed plans call for the renovation of a elderly care / assisted living facility.
Enclosed is a request for quotation (RFQ) to supply and install Custom Cabinets & Counter Tops per the attached scope of work. Please be advised that attendance at the site visit is required prior to submitting a bid. If your company would like to be considered for this project, please review the attached scope of work, scheduled the site visit scheduled with the Project Coordinator (Dan Blair 607-793-7071). This is an RFQ (request for quotes). This is not an IFB (bid). The Broome DDSOO intends to award the contract to the most responsible and responsive vendor that provides the lowest cost and meets the requirements detailed in the scope of work. Upon request, vendor must demonstrate relevant experience on prior projects of similar magnitude. The Broome DDSO reserves the right to reject any and all quotes. In deciding whether to submit a quote, please be aware the awarded vendor must adhere to time frames outlined in the scope of work. The awarded vendor will also be required to submit certificates of Liability Insurance and Worker's Compensations Insurance as well as completed Lobbing Law Forms and Signed Appendix A. Please be prepared to submit these in a timely fashion. The Broome DDSO will make no concessions to the vendor for any alleged misunderstandings or deception because of quantity, quality, character, or other conditions. The Broome DDSO's interpretation of specifications shall be final and binging. Please note all staff working at the project site will be required to sign an attendance sheet for record purposes. This includes the residence visitors log and/or an attendance sheet provided by the maintenance department, all bids must be submitted on bid forms provided. This is a NYS Public Works Project and Prevailing Wages MUST be paid. A New York State Department of Labor Prevailing Wage Request is submitted on behalf of OPWDD as the contracting agency. This project will be assigned an Original Wage Schedule with an assigned Prevailing Rate Case number. Selected vendor must comply with all prevailing wage regulations distributed by the NYSDOL Bureau of Public Work. Certified payroll record MUST be submitted at the time of invoicing. **If you need assistance understanding prevailing wages. Please contract NYS Dept. Of Labor @ 607-721-80.
