Offerors shall submit one (1) original and four (4) copies of the sealed proposal documents in response to this Request for Competitive Sealed Proposal (RFCSP). o Base Proposals including Contingency Allowance and all other required information shall be submitted no later than Friday, November 18, 2022 by 11:00 AM. o Subcontractors List, Schedule of Values and HUB Certificates are to be submitted with the Proposal Form. This supplemental information must be clearly marked with the name of the project and the name of the General Contractor Bidder if submitted separate from the Proposal form. Failure to provide the Subcontractors List, Schedule of Values and HUB Certificates prior to the deadline will result in zero (0) points for evaluation criterion #5. o If submitting a one-time deduct, you must use Appendix "B" provided with this RFCSP. All questions regarding the plans or specifications for this project shall be directed to the Architect/Engineer. Any changes to the proposal must be executed by an addendum. A certified or cashier's check, or bid bond to be submitted with Pricing Proposal acceptable to Owner, in the amount of at least five percent (5%) of the largest amount bid must accompany each Proposal submitted.
