A valid Bid Bond (equal to 5% of the Base Bid) A valid WV Contractor License List of Subcontractors and Suppliers completed and signed State of WV Purchasing Affidavit completed and signed State of WV Drug Free Work Place Affidavit completed and signed Certificate of Receipt of Addenda completed and signed Envelope #2 Form of Proposal completed and signed Bidders will provide an electronic convenience copy to ZDS Design onsulting Services, via e-mail to contact@zdsdesign.com after the Bid opening. The Owner reserves the right to reject any or all bids, and the right to withhold final award of contract for sixty (60) days after opening bids. The goal is to issue a Letter of Intent for Notice to Proceed within 45 days after bid opening. This Contract will be subject to liquidated damages in the amount of one-thousand dollars ($1,000.00) per day for failure to achieve substantial completion indicated in contract documents. The successful Bidder will be required to execute a contract, comply with all rules and regulations of the West Virginia Workers Compensation Laws, and provide evidence of insurance coverage to the minimum limits required by the contract documents. All Prime contractors shall include their West Virginia Contractor License Number in their Bid Proposal. All Prime contractors bidding the project are encouraged to attend the project sites to familiarize themselves with the project locations, site conditions and other relevant information. All site visits shall be scheduled through Raleigh County Schools and shall occur after the pre-bid and prior to the bid opening with additional details for site visits provided during the prebid conference. The Contractors shall provide Builders Risk Insurance for the total value of the Bid. All Contractors and Subcontractors working on Independence Middle School shall be subject to Davis-Bacon wages, rules, and regulations; however, this does not apply to the other County Funded schools
