On behalf of the Daniels Memorial Healthcare Center, the City of Scobey, Montana is soliciting Proposals from qualified architectural firms to assist in preparation of a Preliminary Architectural Report (PAR), final project design and inspection, financial feasibility, environmental review, and other architectural / engineering services related to site location and construction of a new facility to house the Healthcare Center. Services may also include grant writing, grant administration, preparing final construction design and bid documents, assistance in project bidding and contractor award, monitoring construction activities, other architectural / engineering services, and warranty phase for USDA Rural Development requirements as deemed necessary. Minority and women's business enterprises and labor surplus area firms (as determined by the U.S. Dept. of Labor) are encouraged to apply. INSTRUCTIONS TO APPLICANTS / SUBMISSION PROCEDURES All questions and contacts regarding this RFP must be directed to Kody Brinton, at kbrinton@dmha.net. Please limit contacts to this source (the owner's representative). Respondents will need to submit five (5) copies of their response to this Request for Proposals (hereafter "RFP"). Responses to this RFP are to be delivered in one box or envelope. The outside of the box or envelope should be clearly labeled on the front with the words: Response to Request for Proposals - City of Scobey, Montana and indicate if the proposal is applicable to all components of the proposed scope of work or only particular parts or elements. Background Daniels Memorial Healthcare Center plans to build a new one-level Critical Access Hospital, not to exceed (25 beds) and Rural Health Clinic in Scobey. The existing hospital was built in phases starting in 1950. The last major renovation occurred in 1997. The hospital facility has reached the point where continued repairs are no longer cost effective. Preliminary space estimate for a new facility at a maximum of 48,000 square feet. Needs include enclosed ambulance entry, emergency department for trauma care and observation, exam rooms for four providers, procedure space, physical therapy with hydrotherapy, imaging and laboratory space. Scope of Work The selected consultant would be responsible for preparation of a Preliminary Architectural Report according to the requirements of the Montana Department of Commerce and United States Department of Agriculture, Rural Development. In addition, at the pleasure of the City of Scobey and Daniels Memorial Healthcare, the selected consultant or consultants may also be directed to complete final architectural design, including bid document preparation, bid award supervision, project management, construction oversight, and project closeout, including financial feasibility study, site location, environmental review, as well as grant writing and grant administration. Interested firms are invited to submit separate proposals for all or any part of the anticipated scope of work. Anticipated project delivery method will be design/bid/build. The contract will require that the Respondent and all subcontractors comply with all applicable requirements of State and Federal project funding agencies, and meet USDA Community Facility Program requirements. The City of Scobey in cooperation with the Daniel Memorial Healthcare Center reserves the right to award a contract to a single firm or any combination of firms that offer the best desired services related to the proposed project undertaking. Consistent with Section 3 of the Housing and Community Development Act of 1968, regulations applicable to any potential Community Development Block Grant (CDBG) require that to the greatest extent feasible, opportunities for training and employment arising in connection with a CDBG-assisted project will be extended to local lower-income residents. Further, to the greatest extent feasible, business concerns located in or substantially owned by residents of the project area will be utilized. Respondents RFPs will be evaluated according to the following listed criteria and weight of each factor: 1. The qualifications of professional personnel to be assigned to the project - 35 % 2. Related experience on similar projects - 35 % 3. Capability to meet time and project budget requirements - 15 % 4. Location - 5 % 5. Present and projected workloads - 5 % 6. Any past work for the City of Scobey - 5 % Responses should include: 1. The firm's legal name, address, and telephone number. 2. The principal(s) of the firm and their experience and qualifications. 3. The experience and qualifications of the staff to be assigned to the project or particular activity or work element. 4. A description of the firm's prior experience, including any similar projects, size of community, location, total construction cost (if applicable), and name of a local official knowledgeable regarding the firm's performance. 5. A description of the firm's current work activities and how these would be coordinated with the project, as well as the firm's anticipated availability during the term of the project; and, 6. The proposed work plan and schedule for activities to be performed. 7. Any proposed subcontractors should also be clearly identified, and their profiles described, as well. The City of Scobey will utilize a "proposals based" selection process to negotiate a contract with the firm(s) determined to be the most qualified at a price which is determined to be fair and reasonable. Costs will not be discussed until the top ranked respondent(s) has been selected for contract negotiations. Interviews may or may not be held either in person or "on-line" electronically with the top ranked firm(s). The City reserves the right to accept or reject any and all Proposals, or parts of Proposals; waive technicalities; and to make its decision on the basis of merit, appropriateness, and any other factor. Unless all Proposals are rejected or the solicitation is canceled, a contract will be awarded to the Respondent(s) whose Proposal best meets the requirements and criteria set forth in this solicitation.
