Contractor shall provide steam trap survey at the Sheridan VA Medical Center in accordance with the performance work statement. Please provide pricing on attached bid schedule. U.S. Dept. of Labor Wage Determination # 2015-5411 , rev 19 is applicable to this acquisition and resulting contract. See Section C6 for Subcontracting Plan. Failure to complete subcontracting plan will be deemed noncompliant to the RFQ. The FORM must be filled-out and signed by all vendors. All questions must be received by 1:00 PM MST on 05/02/2023. An amendment will be issued to provide response by 05/09/202 PERIOD OF PERFORMANCE: Services required herein shall be for the ESTIMATED following period: BASE YEAR: May 31, 2023 - May 30, 2024 OPTION YEAR 1: May 31, 2024 - May 30, 2025 OPTION YEAR 2: May 31, 2025 - May 30, 2026 OPTION YEAR 3: May 31, 2026 - May 30, 2027 OPTION YEAR 4: May 31, 2027 - May 30, 2028
