The sealed envelope should be marked "Concrete work Gilbert K8". Items to submit in the bid: 1. A written proposal based on specifications given at the mandatory pre-bid meeting. 2. West Virginia Contractors License 3. West Virginia Business Registration Certificate 4. Proof of Insurance (General Liability, Workers Comp., etc.) with Mingo County Schools listed as additional insured. 5. West Virginia Form 73 "Drug Free Workplace Conformance Affidavit The successful bidder will be required to agree to the terms and conditions similar to WV Form 96 and execute a written agreement. For additional information please contact Mr. Williams Hensley, Director of Facilities at 304-928-7961. Mingo County Schools reserves the right to reject all bids and to waive any informality in the bid process.
