The proposed work provides for design services for a new sidewalk in the City of Edwardsville. The proposed sidewalk will be a minimum of 5 feet wide, approximately 1,500 feet in length and meet all ADA requirements. The proposed work will provide the design for a new sidewalk along the west side of S. 104th Street from Metropolitan south to the Elementary School. The following items should be addressed in your response: 1)Describe the Firm offering to provide services, including type of business, date business was established, type of services provided, and description of client base. Also, identify staff that will be assigned to this project and their background experience, education, and skills necessary to perform the required services; 2)Description of Project Approach by the consultant. Description of Similar Projects. Describe previous projects with similar needs. 3) Provide a minimum of three references of clients where services were similar to those requested for sidewalk projects; 4)Description and Qualifications of Firm's Personnel. Describe qualifications of personnel who would be assigned to this project. Provide resumes of the individuals who would be assigned to the project, including information on their experience with work similar to this sidewalk project; 5) Fee Schedule. Outline the anticipated planned hour allocation for staff time that your firm is expected to use for this engagement. Included must be an itemized fee schedule for all work included for the design of the sidewalk, including a total project cost for design. 6)Availability. Indicate the firm or individual's availability to work on this project. The City of Edwardsville, Kansas, reserves the right to accept or reject any and all proposals, in whole or in part, and to waive technicalities. Bids may be held by the City of Edwardsville, Kansas, for a period not to exceed 30 days from the date of the opening of the bids for the purposes of reviewing and investigating the bidders' qualifications, prior to awarding the contract. No bidder may withdraw his bid during this period. Instructions to Bidders 1. Each Proposal submitted shall be addressed to the Public Works Director, City Hall, 690 South 4th Street, Edwardsville, Kansas 66113. Proposals shall be delivered to the City on or before the time and date specified in the Invitation for Bids. 2. No bidder may submit more than one Proposal. Two proposals under different names will not be received from one firm or association. 3. No bidder may withdraw his Proposal for a period of forty-five (45) days after the date and hour set for the opening of bids. A bidder may withdraw his Proposal at any time prior to such date and hour, by written request of the same person or persons who signed the Proposal. 4. The filing of any bid shall constitute an agreement of the bidder to the terms and conditions of these Instructions to bidders. 5. Bids received after the specified time of closing will be returned unopened. 6. Each bidder shall sign the proposal using their usual signature and giving the full business address. Additionally, bidder shall initial each and every page of the bid including the Instruction to Bidders and Bid Specifications demonstrating their knowledge and acceptance of all bid terms and specifications. 7.
