Renovation of a residential development in Fairmont, West Virginia. Completed plans call for the renovation of a residential development.

The Fairmont-Morgantown Housing Authority (FMHA) is soliciting bids from qualified companies interested in providing installation of new LVP at 224 Norval Street located in Fairmont, WV. The public housing program provides affordable rental options for low-income families and is primarily funded by the U.S. Department of Housing and Urban Development (HUD). Responses are solicited for this service in accordance with the terms, conditions, and instructions set forth in the RFP guidelines. All questions and requests for accompanied site visits shall be directed to Mark Malone at mmaloneMmhousinq.com 304-216-6519. The Request for Proposals document may be obtained from FMHA central office upon request or through FMHA web site at www.fmhousinq.com,. Request for Proposal documents will also be mailed upon request; however, FMHA is not responsible to ensure timely delivery of mailed Request for Proposals documents. Receipt of responses does not bind the FMHA to any contract for said services, nor does it guarantee that a contract for the Project will be awarded. This project unit is located at 224 Norval Street within Fairmont, Marion County, WV. The project site is located within the city limits of Fairmont. Scope of Work: Installation of Natural Oak LVP (that includes adhesive for LVP) throughout the one-bedroom unit that is approximately 670 square feet. color- (to be selected by FMHA), 2mm thickness plank square edges glue down urethane finish Underlayment to be installed (floor leveling) 4'x5' 6 MM 5 ply Transition Materials to be installed Cove Base to be installed (that included adhesive for Cove Base) o color (to be selected by FMHA to match selected LVP) In the bathroom of the unit, remove and replace the toilet with a new wax ring and bolts (supplied by contractor) Contractor is responsible for removal and disposal of all trash and materials removed. Contractor shall be responsible to provide all materials and labor to complete project. Contractor is responsible for traffic control during the project. Contractor is responsible safe work practices when performing this work. Contractor is responsible for obtaining a City of Fairmont Building Permit. FMHA reserves the right to: (1) evaluate the proposals submitted; (2) waive any irregularities therein; (3) accept any proposal (4) reject any or all proposals. The selected company shall provide FMHA the following within five (5) business days of notice of acceptance of proposal: State of West Virginia Contractors License State of West Virginia Business License City of Fairmont VVV Business License Proof of Workers Compensation for all employees working on project (Waivers issued by the State of WV will not be accepted). Proof of Liability Insurance Completed W-9 This shall be a fixed price proposal including all elements of the scope of work. The invoice will be submitted after the work is completed at each determined unit. The FMHA is tax exempt so proposals shall not include sales tax. The unit will be vacant effective October 5th, 2023, and the selected company can begin the work on October 13th, 2023. The necessary flooring work (scope of work) shall be completed by October 23rd, 2023.
