Bridge 25530 is a timber structure that is located over a dry run on CSAH 7 approximately 2.8 miles north of TH 19. During a routine inspection, extensive decay was found in the east pier cap over piles 4 and 5. The bridge is currently load posted at 32 ton and after a structural review it has been determined that this posting could be maintained with repairs to the east pier. Goodhue County is requesting a quote to repair the east timber pier cap in accordance with the attached plan and the following parameters: Provide certified welders. o Work may be done under traffic. o Failure to complete the work in the allotted time will be subject to a $100.00 liquidated damage assessment for each additional calendar day. o Provide proof of insurance meeting the attached insurance requirements. Work quoted, if approved, must begin within 8 calendar days of quote approval and shall be completed in 5 working days. Please provide a unit price for the items listed on the next page which should include all costs associated with this work. INSURANCE REQUIREMENTS The Contractor shall not commence work under the Quote until they have obtained the following insurance, and Goodhue County has approved such insurance. The Contractor shall deposit with the County Engineer, Certificates of Insurance verifying the coverage and limits, as applicable to this project, of the following insurance policies. All policies and certificates shall provide that the policies shall remain in force and effect on 30 days written notice to the County Engineer before cancellation. The Certificates of Insurance shall be submitted at the same time as the Quote. The Contractor shall procure and maintain during the life of the Quote and until all work required in the Quote has been fully accepted, insurance policies as follows: (A) PUBLIC LIABILITY AND PROPERTY DAMAGE INSURANCE: For and in behalf of the Contractor, and the County of Goodhue as joint assureds, and with a cross liability endorsement protecting the County of Goodhue from claims or damages for personal injuries, including accidental death, as well as for claims for property damage which may arise from operations under the Quote, whether such operations be by the Contractor or by a Subcontractor or by anyone directly or indirectly employed by either of them. Said Public Liability and Public Property Damage Insurance Policy shall provide that the insurance company waives the right to assert the immunity of the County as a defense to any claims made under said insurance. The amount of such insurance will be as follows: Public Liability Insurance in an amount of not less than two million dollars ($2,000,000.00) for all damages arising out of bodily injuries to, or death of, one person and subject to the same limit for each person in a total amount of not less than two million dollars ($2,000,000.00) on account of one accident, and property damage insurance in an amount of not less than two million dollars ($2,000,000.00) for all damages to or destruction of property in any one accident and subject to that limit, a total limit of two million dollars ($2,000,000.00) for all damages to or destruction of property during the policy period. (B) WORKER'S COMPENSATION INSURANCE: For all their employees employed at the site of the project and, in case any work is sublet, the Contractor shall require the Sub-contractor to provide Worker's Compensation Insurance for all their employees. (C) AUTOMOBILE PUBLIC LIABILITY INSURANCE: Two million dollars ($2,000,000.00) for all damages arising out of bodily injuries to, or death of one person, and subject to that limit for each person, a total of two million dollars ($2,000,000.00) for any one accident, and property damage liability insurance in an amount of not less than two million dollars ($2,000,000.00) for all damages to or destruction of property in any one accident and subject to that limit, a total of two million dollars ($2,000,000.00) for all damages to or destruction of property during the policy period, if any motor vehicles are engaged in operations within the term of the Quote on the site of work covering the use of all such motor vehicles.
