General Requirements The contractor is responsible for seeping and all dust control The contractor is responsible for work zone traffic signs in compliance with MUTCD standards The contractor is responsible for marking all work areas/job sites and contacting dig-safe. The contractor will be responsible of all damages to utilities and structures caused by construction operations Town of Lyndeborough Requirements Provide traffic control Final walk-through of project and acceptance of work done Notify residents of work area in advance of project starting date If needed, add gravel to the base after reclaiming is complete, if original base material is insufficient. This will be done prior to final grade/shape, if needed Will back up all pavement edges and driveway opening as needed Please provide the total costs of completing each project; including all required materials and labor. It is the intention of the Town to select one contractor to perform all five (5) projects. The selected contractor shall give four (4) weeks' notice of starting paving date to allow for any culvert replacement or road work needed prior to paving. All five projects shall be completed by September 1, 2024. Quoting should be broken out by individual project. The selected contractor will be required to provide insurance documentation acceptable to the Town of Lyndeborough, including but not limited to; proof of liability insurance and workmen's compensation insurance and W-9. Payment will take place upon successful completion, inspection and acceptance by the Road Agent.
