2024 Lead Service Line Replacement Equity Prioritization Program All work, including site restoration, must be completed by December 27, 2024. Liquidated Damages, per diem: $935.00 The SBE requirement for this project is 25% of the contract base bid. A description of the scope of work to be performed by the SBE on this project; The prime contractor shall be credited for the entire expenditure to SBE firms only if all of the identified scope of work is performed directly by the certified SBE firm. proposes to furnish work, material, labor and services as set forth in the description in the Invitation to Bid at and for the prices hereinafter named according to the provisions in the Official Notice and the Invitation to Bid, Detail Specifications, Special Provisions, Addenda’s, if any, and Plans for this particular project on file in the office of the Commissioner of Public Works, and if successful, hereby agrees to enter into a contract with the City of Milwaukee with such sureties as required and set forth in the aforesaid documents for the performance of said contract (said contract form being on file in the office of the Commissioner of Public Works), and in accordance with the terms and conditions set forth in the contract documents, to-wit: written agreement, official notice, invitation to bid, bid, instructions to bidders, detail specifications, special provisions, special conditions when applicable, plans, schedule of fixed prices, supplemental agreements and all addenda. In case of obtaining the award, the undersigned bidder anticipates using, subject to the approval of the Commissioner, the following subcontractors, with the class of work to be performed by each, which list shall not be added to nor altered without the written consent of the Commissioner: The bidder's commitment for SBE participation on this project is 25 % The bidder's commitment for Resident Preference Program Participation on the project is 40 % The bidder's commitment for Apprenticeship Program participation on this project is: Apprentice(s) from each of the following trade(s) are to be employed: Construction Craft Laborer Operating Engineer Plumbing For those bids being submitted through Bid Express and using a manual Bond, the contractor may send a signed/executed copy of the bid bond to the Department of Public Works Contract Office by emailing DPWbids@milwaukee.gov All bid documents for the listed project(s) are available electronically at www.city.milwaukee.gov/dpwbids and at www.bidexpress.com and can be submitted using either Bid Express or by emailing PDF copies of the complete bid documents to DPWbids@milwaukee.gov before 10:30 am on the date of the bid opening. Any required addenda or response related to listed projects will be posted on both the DPW and Bid Express websites. Please contact the Office of Equity and Inclusion (OEI) at 414-286-5553 should you have any questions or concerns regarding the training or reporting process Work may include, but is not limited to, the procurement of all necessary permits, a site visit prior to work beginning, scheduling the installation with the property owner, filter distribution (contractor to obtain filters from MWW), traffic control, erosion control, saw-cutting, excavation, installation of a new copper service line utilizing trenchless technology (MWW approval is required for open cut installation), tapping of the water main, abandonment of the lead service line, installation of an updated water meter setting up to and including the outlet valve, reconnection to the interior building piping, code compliant restoration of the electrical ground wire, backfilling of the excavation(s), restoration of pavement, curb and gutter, and walk, and final cleanup of the work site. Temporary and permanent restoration requirements will be at the discretion of MWW. The contractor will be required to complete a minimum of 2 service replacements per working day. Project locations will be as shown on the project Site Map and the contractor may be assigned replacement projects in one (or more) of the numbered work areas. All work shall be in compliance with the City of Milwaukee Rules and Regulations Governing Water Service, the City of Milwaukee Water Service Piping Specifications, and the City of Milwaukee Special Provisions for Lead Service Line Replacement – Equity Prioritization Program. Prices from all bidders will be kept on file. If the successful bidder fails to fulfill the contract requirements, an agreement with a secondary contractor may be pursued for work at specific emergency locations.
