Oak Creek Skate Lift Station Project Location 2000 Mill Rd., South Milwaukee, WI 53172 Contract Description: Replacement of sanitary lift station and force main at the Oak Creek Warming Building located on Mill Pond. Substantial Completion Date: 11/29/2024 Project funded by local. Milwaukee County project bid summary results will be posted online. Bid summary results may be available a few hours following the Bid Due Date time. Contractors shall employ a minimum of 50% of the Contract labor force with employees that reside in Milwaukee County. Documentation must be performed by LCP tracker Labor Compliance software. All bidders must meet the minimum software requirements online and be able to view, enter, scan and upload PDF files to submit a bid. For further information contact 414-278-4861. Targeted Business Enterprise (TBE) participation goal for this project is 25% percent. To Be Considered for This Project All Required TBE-14 / DBE-14 Forms Shall Be Completed and Signed by Prime and Submitted as Indicated in Invitation to Bid. Bidder's attention is called to the "Notice of Requirements for Affirmative Action to Ensure Equal Employment Opportunity" (Executive Order 11246), contained in the Bid Documents, which set forth, in part, goals and timetables for the employment of women and minorities in the project area. Bidders shall submit a completed contractor qualification statement not less than five(5) calendar days prior to the bid due date. Contractor qualification statement is valid for a three-year period commencing on the date accepted. Refer to Milwaukee County Ordinance Chapter 43 for more information. Bidders shall submit the completed Subcontractor-Supplier Information Entry with the Bid. Failure to submit this information with Bid may result in rejection of the Bid as non-responsive. A 10 percent bid bond or certified check is required with bid. The low responsive, responsible bidder will be recommended for contract award. Owner reserves right to reject bid, to waive informalities in bid or to accept bid which will be in best interest of Owner. Base Bid shall remain open to acceptance for a period of 60 days after bid due date. SCOPE OF WORK GESTRA has performed the following services for the project: Contacted Diggers Hotline to locate the public utilities at the site prior to drilling. Performed a site meet with Milwaukee County Parks locator for locating Milwaukee County Parks utilities. Boring locations were marked in the field by GESTRA by using tape and stake methods. Elevations and coordinates of the boring locations were determined using a Geomax Zenith 35 GNSS-INS (GPS) receiver were referenced to Wisconsin County Coordinate System (WCCS), Milwaukee County. Location and elevations were not obtained by a licensed surveyor. Obtained Street Opening Permit from the City of South Milwaukee to perform a boring on Minnesota Avenue/7 th Avenue.
