Site work for a religious / funeral development in Athens, Ohio. Completed plans call for site work for a religious / funeral development.

Submit all questions regarding this RFQ in writing to Brandon S. Sturgill at Brandon.sturgill@mha.ohio.gov with the project number included in the subject line (no phone calls please). The Ohio Department of Mental Health and Addiction Services currently operates six regional hospitals on different campuses. Currently, as many as four of these locations contain an on-site cemetery used to bury patients of the hospital that died. The cemeteries are maintained to varying degrees with many grave sites still unidentified or with headstones that carry only a number. The Department is seeking design work to help bring the cemeteries up to a higher level of appeal and dignity for the individuals buried at the sites and for families, friends and visitors. Full design services including Schematic, Design Development, Construction Documents, Construction Administration and Closeout services. During the construction period, provide not less than 4 hours (excluding travel time) on-site construction administration services each week, including (1) attendance at progress meetings, (2) a written field report of each site visit, (3) on-site representation comprised of the A/E and its consultant staff involved in the primary design of the project, all having relevant and appropriate types of construction administration experience.
