The equipment in need of repairs is a Cleaver Brooks boiler, model # FLX-200-1200-160HW Serial # BT6784. This boiler is known and referred to as boiler #2. Contractor to provide complete boiler retube on the FLX boiler specified. o The boiler tubes are to be Cleaver Brooks 1-1/2" OD x .095 SA178A. Qty: 231 tubes. Tubes must meet OEM specs at a minimum. o Contractor to disengage power from the main breaker, close all gas valves and perform lock out/tag out per NFPA-70E procedures. o Contractor to test boiler for any gas leaks before the initiation of any work. o Contractor shall remove the boiler side panels; this includes insulation. Contractor to expose all boiler tubes and remove all boiler tubes. o Contractor to inspect the interior of the boiler after tubes have been removed. USP LEE personnel will assist contractor in inspecting for any damages prior to the new tubes being installed. USP LEE personnel will grant the contractor permission to continue the assigned work after their satisfied inspection. o Contractor to install all new tubes in the correct orientation and order per manufacturer. o Contractor to perform hydrostatic testing upon successful tubes re-installation. o Contractor to re-install existing insulation utilizing per manufacturer specifications. o Contractor to re-install boiler sides with new gasket material from the manufacturer. o Contractor to repair any broken studs throughout the repair. o Contractor is responsible for the completion of the necessary paperwork to ensure the boiler pressure vessel is certified per National Boiler Code. A National Board R Certificate plate shall be affixed to the boiler skin upon completion.
