i) Road cut reinstatement shall be undertaken by resurfacing with at least two lifts of asphalt; one lift of Performance Grade Superpave 12.5mm Level B (PG 58-34) asphalt base and one lift of Performance Grade Superpave 12.5mm Level B (PG 58-34) asphalt surface, unless otherwise specified by the Contract Administrator to the same thickness as the adjoining construction. The minimum lift thickness shall be 50 mm for each lift, the maximum lift thickness shall be in accordance with City special provisions referenced in Section E of this document. ii) All road cuts must be saw cut to the marked dimensions supplied by the Contract Administrator. The contractor is not permitted to saw cut beyond the marked dimensions without the prior approval of the Contract Administrator. The asphalt shall be laid in accordance with the City Standards. Base coat shall be placed such that it is ramped at the edges of the cut to provide a vertical face at the edges of 10-12 mm. The Contractor shall apply a bond breaker between base course asphalt and any temporary ramped material to facilitate removal of the ramped material prior to final asphalt. Tack coat shall be applied to the edges prior to placing the final lift of asphalt. iv) The road cut shall be reinstated to the same level as the adjacent surface and the riding surface shall be to the satisfaction of the Contract Administrator. v) Granular materials used for backfill purposes shall conform to the City special provision F-3147. vi) No frozen material shall be used to backfill any road cut. vii) No rocks larger than one hundred and fifty millimeters (150 mm) across in any direction shall not be used to backfill any road cut. viii) No material having a moisture content too high to enable the required percent compaction to be achieved shall be used as backfill material. ix) All material shall be placed and compacted in lifts that enable the compaction equipment being used to obtain the required compaction throughout the entire backfill depth. x) All compaction shall be done by mechanical means. xi) Granular material shall be compacted to a degree of compaction not less than one hundred percent (100%) of maximum dry density as determined by OPSS 501. xii) Granular "A" and asphalt bases shall conform to all applicable OPSS provisions. Commercial General Liability insurance issued on an occurrence basis for an amount of not less than $5,000,000.00 per occurrence for all sums which the Contractor shall become obligated to pay by reason of liability imposed by law upon the Contractor for damages arising out of or in connection with all operations of the Contractor, its agents, officers, employees or other persons for whom the Contractor is legally responsible relating to their obligations with the project. Such insurance shall include, but is not limited to, bodily injury, death and property damage including loss of use: premises, property and operations liability; products and broad form completed operations liability; blanket contractual liability; cross liability; severability of interest clause; contingent employers liability; personal injury liability; owner's and contractor's protective coverage; non-owned automobile liability; broad form property damage; occurrence property damage; employees as additional insured, medical payments.
