To establish an Indefinite Quantity Agreement for Gymnastic Equipment Preventative Maintenance & Repair Services on an as needed basis. The estimated dollar amount to be spent on this agreement is up to and not to exceed $11,000.00. This is an estimate of the annual needs of the City under this Agreement and are for bidding purposes only. This estimate is not to be construed as representing an actual order for that amount, or a guarantee that any minimum amount will actually be purchased. Subsequent to the acceptance of an offer, individual written purchase orders may be issued as needed by the City to purchase items listed herein during the term of the agreement. At no time shall the obligation of the City agency exceed the dollar amount of any associated purchase order The Agreement will expire on 03/31/2026. Any available funds not obligated by the City by means of a Purchase Order on or prior to that date shall be cancelled after that date. See the attached Exhibit A describing the maintenance service needed for each piece of gymnastic equipment: For each line please provide the unit cost as requested. These additional line items will be made available to the City at the pricing submitted through Indefinite Quantity Agreement. Bidder must add any additional charges and fees not covered in the lines. Any charges not specifically listed here will not be approved for payment, should a purchase order be awarded. Prices shall be FOB Destination Freight Prepaid & Allowed unless otherwise specified. RESPONSIBLE WAGE: Contractor shall pay a responsible wage to all employees directly performing the work specified in this contract. The responsible wage is a minimum of $20.00 per hour. HEALTH INSURANCE BENEFIT: Contractor shall provide an adequate and affordable health insurance benefit to its employees. The benefit must be provided as part of an overall compensation plan and the benefit cannot be limited to a specific project. The health insurance benefit is adequate and affordable if it meets both the minimum value and affordability requirements established in rules promulgated pursuant to Public Law 111-148, The Patient Protection and Affordable Care Act or a successor to that law. The benefit must otherwise meet the requirements of a bona fide fringe benefit, as defined in 29 CFR 4.171 or a successor to that section. An employer may provide a health insurance benefit through the Small Business Health Options Program, pursuant to Public Law 111-148, so long as it otherwise meets the criteria of this definition.
