**As of April 7, 2025, the project was awarded to DC Construction Services, Inc but the awarded amount is not available.** The City of Anderson reserves the right to delete any section(s) of the bid proposal. Said materials, supplies, and work to be furnished or performed shall comply with the requirements as may be designed by the Board of Public Works which shall include the City's right to have an independent testing laboratory test asphalt materials for liquid asphalt content. Any bid received after designated time will be returned unopened. Bids will then be taken under advisement for review by the owner and architect/engineer. The bid shall consist of the following properly and completely executed documents: 1)Bid shall be submitted on the enclosed Itemized Bid Form. 2)Bid shall be submitted on General Bid Form 96 as prescribed by the Indiana State Board of Accounts. 3)Each bid proposal shall include a financial statement, a statement of experience, proposed plan or plans for performing the public work, and the equipment available for the performance of the public work. 4)Each bid proposal shall include a properly executed non-collusion affidavit as required by the statues of the State of Indiana. 5)Bid Security: Pursuant to Indiana Statute I.C. 36-1-9-6, a bidder shall file a certified check or bid bond with each bid, in an amount equal to ten percent (10%) of the total bid proposal, as evidence of good faith that the successful bidder will execute within ten (10) consecutive calendar days from acceptance of a bid proposal, a contract as set out in the specification. 6)City of Anderson Employment Eligibility Verification. 7)Bidder's written plan for a program to test Bidder's employees for drugs in accordance with IC 4-13-18. 8)City of Anderson certification of Non-Segregated Facilities. 9)City of Anderson Non-Discrimination Clause. 10)City of Anderson Workforce Breakdown Forms. 11)City of Anderson Affirmative Action Checklist. 12)All Bidders must be pre-qualified for Indiana Department of Transportation work (INDOT). 13)All contractors, suppliers and subcontractors shall demonstrate 5 years of progressive experience in work similar in nature to this project. All workers employed on the project shall have documented legal citizenship or immigration status. Any general or subcontractor that has outstanding fees or inspections or has outstanding work-related, performance, or quality issues with the City of Anderson may be deemed as non-responsive or non-responsible. Failure of bidder to enter into a contract, execute same and furnish acceptable labor and materials and performance bond as hereinafter required, shall be cause for forfeiture of certified check or bid bond for liquidated damages to the City. The contractor must conform to the antidiscrimination provisions of I.C. 5-16-6, et. Seq. The performance bond and labor and material payment bond shall be for one hundred percent (100%) of the contract amount and shall accompany the executed contract. Bonds shall stay in effect for twelve (12) months after completion and acceptance of the project. Bidder will not be permitted to withdraw his bid for (60) days from the date of bid opening. The need for insurance protection against theft of or damage to materials or contractor's tools or equipment at the job site shall be the bidder's decision and expense, exclusively. The Board of Works reserves the right to waive informalities or reject any and all bids. The City of Anderson by and through its Board of Public Works. This project shall consist ofthe milling, resurfacing, and other asphalt maintenance in the project areas. The streets listed on said proposal may be added to or deleted from during the course ofthe paving season. The contractor shall furnish all materials, equipment and labor, including flagmen, signs, barricades and flashers. The contract unit price times the actual quantities used shall constitute full payment for the work performed. No additional maintenance or any other work incidental to the paving. The paving limits for The Project Areas shall be either marked or designated by the City ofAnderson Engineering Department as shown on the plans. The paving sequence shall be furnished and governed by said Engineering Department and updated periodically as required by utility company work or other traffic changes. Various roads in Anderson.
