Renovation of an elderly care / assisted living facility in Zionsville, Indiana. Completed plans call for the renovation of a elderly care / assisted living facility.

Any Bid received after the designated time will be rejected and returned to the Bidder unopened. In general, the work for the Project consists of, but is not necessarily limited to, the following: ENTIRE ROOF REPLACEMENT, all as required by the Bidding Documents, the Agreement between Owner and Contractor, the General Conditions and any special conditions contained in the Bidding Documents, Specifications and Drawings (collectively the Agreement). The Project is being undertaken by the Owner to benefit the Zionsville Meadows Nursing Home and the public. The Project is a unified bid and will be awarded to a general contractor with a complete bid of all work as spelled out on the documents. Bidders shall assure that they have obtained complete sets of drawings and Contract Documents and shall assume the risk of any errors or omissions in bids prepared in reliance on incomplete sets of drawings and Contract Documents. Bids must be submitted on the forms found in the Bid package, must contain the names of every person or company interested therein, and shall be accompanied by: (1) Revised Form No. 96 as prescribed by the Indiana State Board of Accounts and as required in the Instruction to Bidders, including a financial statement, a statement of experience, a proposed plan or plans for performing the Work and the equipment the Bidder has available for the performance of the Work. (2) Bid Bond in the amount of ten percent (10%) of the total Bid amount, including alternates with a satisfactory corporate surety or by a certified check in the amount of ten percent (10%) of the amount of the total Bid. The Bid Bond or certified check shall be evidence of good faith that the successful Bidder will execute within fifteen (15) calendar days from the acceptance of the Bid, the Agreement as included in the Bidding Documents. The Bid Bond or certified check shall be made payable to the Owner. Any Bid may be withdrawn prior to the scheduled closing time for receipt of the Bids, but no bidder shall withdraw its Bid within sixty (60) days after the actual opening of the Bids. All Bid Bonds and certified checks of unsuccessful bidders will be returned by the Owner upon selection of the successful Bidder and execution of the Agreement, and provision of the required Performance Bond and Payment Bond. The Contractor must conform to the antidiscrimination provisions of Ind. Code 5-16-6 et seq. A Performance Bond with good and sufficient surety, acceptable to the Owner and Engineer, shall be required of the successful Bidder in an amount equal to at least one hundred percent (100%) of the Contract Price, conditioned upon the faithful performance of the Agreement. The Contractor shall execute a Payment Bond to the Owner, approved by the Owner and Engineer and for the benefit of the Owner, in an amount equal to one hundred percent (100%) of the Contract Price. The Payment Bond is binding on the Contractor, their subcontractors, and their successors and assigns for the payment of all indebtedness to a person for labor and services performed, material furnished, or services rendered. The Payment Bond must state that it is for the benefit of the subcontractors, laborers, material suppliers, and those performing services. The surety of the Payment Bond may not be released until one (1) year after the Owner's final settlement with the Contractor. All out-of-state corporations must have a certificate of authority to do business in the State. Application forms may be obtained by contacting the Secretary of State, State of Indiana, Statehouse, Indianapolis, Indiana 46204. The successful Contractor will be required to enroll in and verify the work eligibility status of all employees hired after the date of this Contract through the E-Verify program. The Contractor and Subcontractors will be required to sign an affidavit concerning employment of unauthorized aliens and compliance with State laws. A copy of the affidavit is included in the Bid Documents. The Owner reserves the right to reject and/or cancel any and all Bids, solicitations and/or offers in whole or in part as specified in the solicitation when it is in the best interests of the Owner in accordance with IC 5-22-18-2. The Owner also reserves the right to waive irregularities in any Bid, and to accept any Bid which is deemed most advantageous to the Owner. SCOPE OF WORK: a. We will mobilize on site, set up safety, equipment and material. b. We will remove and dispose of the two existing roofing systems down to the concrete roof deck. c. We will inspect the concrete roof deck and report any damage or deterioration to the owner. d. We will prime the concrete roof deck per the manufacturers specifications. e. We will install a self adhered vapor barrier over the concrete deck and envelope up around all roof penetrations and perimeter walls. f. We will install one layer of two inch insulation over the concrete roof deck and secure with manufacturer standard insulation adhesive. g. We will install 1/4 " per foot tapered insulation system over the base layer of two inch insulation and secure with manufacturer standard insulation adhesive. h. We will install 1/2 " per foot tapered saddles between the roof drains to promote positive roof drainage and secure with manufacturer standard insulation adhesive. i. We will install tapered sumps around the roof drains to promote positive roof drainage and secure with manufacturer standard insulation adhesive. j. We will install new "RetroFit" roof drain inserts in the existing roof drain assemblies. k. We will install new wood blocking as needed on the perimeter walls. l. We will remove and dispose of the existing perimeter metal wall panels as needed. m. We will install 1/2 " sheathing over the perimeter walls and secure with mechanical fasteners to the existing framing. n. We will install 60 mil white TPO roof membrane over the new insulation system and secure with manufacturers standard TPO bonding adhesive. o. We will install 60 mil white TPO flashings around all existing roof penetrations. p. We will install 60 mil white TPO flashings over the new sheathing on the perimeter walls with intermediate securement per the manufacturers instructions. q. We will fabricate and install 24ga pre-finished metal coping, fascia, and counter flashing as needed. r. We will install TPO walk mat at the roof access point and around all serviceable mechanical equipment. s. We will provide and install pipe support blocking under all existing piping running across the roof. t. We will provide and install one new roof hatch with ladder up post and perimeter guard rail. u. We will conduct a final roof inspection with the manufacturer representative. v. We will provide a two year contractor warranty and a 20 year manufacturer warranty upon completion. Refer to the attached Facility Summary Report for additional information. o This project is a total roof replacement including removal of existing material including rubber, rock ballast, and insulation to the structural decking. Pricing shall be per the attached bid form. o The new system shall be a TPO white roofing system. Approved manufacturers are Carlisle, GAF, Firestone, Mule-Hide, or equivalent. o Contractors are responsible for dumpsters and removal of debris. o Contractors are responsible for storage containers if necessary for supplies. o Contractors are responsible for permits from the City of Zionsville. o Contractors must produce COI prior to contract signing. o All submittals must be submitted and approved before the contract signing. o All questions should be submitted by email to timcornelius@tcm-bai.com o All contracts and payments will be in AIA formats (refer to sample contract).
