Vendors must invoice for actual cost incurred as described herein. A) Construct a private water line connecting the subject property to the Prince William County Service Authority's (PWCSA) public water line/meter in accordance with the Virginia Uniform Statewide Building Code (VUSBC), and all other applicable state and local laws, regulations, and ordinances. B) Serving as an agent for the property owner, the contractor shall contact PWCSA and fill out all necessary applications to initiate PWCSA's extension of a public water line to the edge of the easement at 6670 Fayette St., Haymarket, VA 20169. Initiation of PWCSA water connection shall include payment of the $5210.00 tap/connection fee. All work and inspections shall be performed in accordance with the attached local policy PWCSA Service Connection Process. C) Upon PWCSA's completion of water tap installation, contractor shall install a water line in compliance with the VUSBC from the point the water line exits the subject property's house foundation to the PWCSA meter/tap. The approximately 40-100 foot private water line will begin at an approximate depth of XX inches minimum exiting the house foundation and terminate at approximately XX inches depth at the PWCSA meter/tap. The residential plumbing code requires that all fixtures have an air gap or vacuum breaker device along the water line prior to entry into the meter/tap. D) Follow all regulations and permitting pertaining to erosion and sediment control including stabilizing, seeding, and grading the site after construction to return to the original state. Control construction runoff with proper practices so as not to become a nuisance to the owner or to neighboring properties or cause sediment to be discharged into state waters and drainage ditches. Any construction debris/necessary brush removal must also be removed from the site and disposed of properly. E) Well Abandonment: Ensure that the connection to public water has been approved and has been placed into operation at this address. Subcontract with a properly licensed water well system provider to appropriately abandon existing private drinking water supply/well. Abandon the well as specified by the Regulations and by the Prince William County Health Department Well Abandonment Permit175-24-0051. This would include: disinfecting the existing well and assessing the well depth, materials, and depth to water. Salvage any materials that can be removed from the well. Additional requirements and conditions may be found on the abandonment permit. Submit the required Uniform Water Well Completion Report GW-5 Well Abandonment Form with all required information filled out. F) Provide to VDH_SWAP_GRANT@vdh.virginia the following information by the listed deadline: o Project Start and End Dates: Estimated project start and completion dates to be provided two (2) weeks prior to the start of the installation/work or within four (4) weeks after issuance of PO (issuance of Notice to Proceed); whichever is earlier. o Name and licensure of subcontractor(s): The contractor shall within four (4) weeks after the signing of the contract (or within two weeks of the work starting or within 1 week after changing subcontractor; whichever is earlier), notify VDH in writing of the names of subcontractors proposed for the principal parts of the work. o Request for Change Orders: All request for changes must be provide in writing per the contract terms and conditions and are not authorized until a response in writing is sent by the SWAP team. o Request for Extensions: If it is determined that the work will not be able to be completed by the contract expiration date listed in the Notice to Proceed and the PO, a request for an extension must be provided, in writing, a minimum of two weeks prior to contract expiration date. Justification for why work could not be performed within original contract term must be provided. Extensions requested on or after the expiration date will not be able to be accommodated. G) Bidders and subcontractors shall comply with all requirements of the Department of Professional and Occupational Regulations (DPOR) for contracting and executing the contract with the Virginia Department of Health. Licensing Requirements for Water Connection: Class A, B, or C Contractor's License from DPOR, plumber's license from DPOR. Licensing Requirements for Well Abandonment: Operate within the requirements of the DPOR Water Well System Provider Certification Operate within the requirements of the DPOR Board for Contractors and proof of insurance.
