The work consists essentially of the following estimated quantities: 1 Mobilization LS 1 2 Install Curb Inlet Protection EACH 63 3 Remove Concrete Header LF 25 4 Subgrade Preparation SY 43,075 5 Construct 7" Concrete Pavement (Type L65) SY 39,175 6 Construct 10" Concrete Pavement (Type L75) SY 3,900 7 Construct Concrete Header LF 240 8 Construct 6" PCC Median Surfacing SF 820 9 Adjust Storm Sewer Manhole to Grade EACH 12 10 Adjust Sanitary Sewer Manhole to Grade EACH 57 11 Construct Curb Inlet - Type I EACH 31 12 Construct Curb Inlet - Type III EACH 27 13 Construct Curb Inlet - Type III (Extra Deep) EACH 5 14 Install External Frame Seal on Sanitary Sewer MH EACH 57 15 Install Seeding - Type B ACRE 100 16 Install Rolled Erosion Control, Type III Seeding ACRE 20 17 Install Permanent Preformed Tape Marking - Type 3, 12" White LF 195 18 Install Permanent Preformed Tape Symbol-Type "BUMP" White EACH 16 19 Traffic Calming Device - Speed Table EACH 6 20 Construct Concrete Median Nose EACH 4 21 Connect to Existing Concrete Pavement EACH 5 22 Install Concrete Washout EACH 1 All proposals must be submitted on bid forms furnished by SEH Engineers, and must be accompanied by a bid bond or a certified check in the amount not less than $175,000.00 made payable to the Treasurer of Sanitary and Improvement District 638 as security that the bidder to whom the contract may be awarded will enter into a contract to build the improvements in accordance with this Notice to Contractor and will give a contract and maintenance bond in the amount of 100% of the total contract price, all as provided in the specifications. Bid bonds must be executed by a duly licensed corporate surety. Checks accompanying proposals not accepted shall be returned to the bidders. No bidder may withdraw his proposal for a period of sixty (60) days after date set for opening of bids. All proposals must be enclosed in an envelope, sealed and addressed to the "Clerk of Sanitary and Improvement District 638" and shall be marked "PROPOSAL FOR PAVING SYSTEM - SECTION I" with the bidder's name and address on said envelope. The Bid Security shall be in a separate sealed envelope attached to the Proposal envelope and shall be marked "BID SECURITY" with the project identified thereon. Proposals received after the above time shall be returned unopened to the bidder submitting the proposal. The work herein provided shall be done under written contract with the responsible bidder submitting the lowest acceptable bid in accordance with the requirements of the plans and specifications and as provided by law. Sanitary and Improvement District 638 of Douglas County, Nebraska, reserves the right to reject any or all bids received and to waive informalities and irregularities.
