Reference Number 0000384688 Willow Point Rehabilitation & Nursing Center - Admin & West Wing Roof Project Locations: 3700 Vestal Road, Vestal, NY 13850 Award Requirements - Reserve Rights to Award by Line Item Bonding Requirements - Bid Bond 5 % - Performance Bond 100 % - Labor and Materials Bond 100 % Each Proposal must be made upon the forms provided by the County and shall be accompanied by a deposit of five percent (5%) of the amount of the bid. Said Deposit shall be made payable to the Broome County Director of OMB, shall be considered a guarantee that the bidder will enter into the Contract with the County if it is awarded to him/her. Each Proposal must be accompanied by a Deposit of Cash, Certified Check or Bid Bond, payable to Broome County Director of OMB, in the amount specified by this advertisement. The retention and disposal of such 1 Cash, Certified Check or Bid Bond, by the County, shall conform to Article 5A, Section 105 of the General Municipal Law, as amended. If the bid price for this project exceeds $100,000, then the Broome County Minority Business Enterprise (MBE) subcontracting goal is 3.5%, and the EEO Minority Hiring Goal is 5.0% in accordance with the programs detailed in the bid documents. Attention of bidders is called to the "Information for Bidders" in the Proposal, Specifications and Contract Agreements. One (1) original, one (1) duplicate and one (1) electronic copy on usb drive of the Contract Proposals must be submitted in a sealed envelope with the words "Broome County Division of Purchasing", the Project Title and DPW Project Number, and the Contractor's name and address plainly printed on the outside. The Bid Number should be placed on the upper left corner of the envelope. Each bid must be in a separate envelope. General Construction Contract (GC) 1. Work in the General Construction Contract includes, but is not limited to the following: a. Staging. b. Selective Demolition. c. Carpentry. d. Thermal and Moisture Protection Systems. e. Openings, exterior. f. Finishes g. Specialties. 2. Temporary facilities and controls in the General Construction Contract include, but are not limited to: a. Temporary facilities and controls that are not otherwise specifically assigned to the HVAC Contract or Electrical Contract. b. Temporary stairs, landings, walkways, and scaffolding system to accommodate foot traffic and general access for all contractors from ground to roof. i. Construct structurally adequate for passage of persons where overhead exposure and risks of falling objects exist. ii. Construct stairs, landings, and walkways using scaffold or shoring framing. Provide wood plank overhead, protective plywood enclosure walls, handrails, barricades, warning signs, lights, safe and well-drained walkways, and similar provisions for protection and safe passage. Maintain in a manner acceptable to the Owner and the Architect/Engineer. Remove when no longer needed. iii. Provide lockable door at ground level to provide unauthorized access. c. Dumpsters and garbage removal for entire project (all Prime Contractors and Subcontractors). d. Temporary Toilets units to be located in proximity to the project area. Provide selfcontained units, and shield toilets to provide privacy. e. Project identification, including wage rates and labor law documentation, and temporary signage. f. Fire Extinguishers: Provide hand-carried, portable, UL-rated, Class A fire extinguishers for temporary offices and similar spaces. In other locations, provide hand-carried, portable, UL-rated, Class ABC, dry-chemical extinguishers or a combination of extinguishers of NFPA-recommended classes for the exposure. i. Comply with NFPA 10 and NFPA 241 for classification, extinguishing agent, and size required by location and class of fire exposure. g. Power supply necessary for any welding required. h. Barricades, warning signs, and lights. i. Security encloser, lockup, and temporary protections for vehicles and pedestrians. j. Environmental protection. i. Provide protection, operate temporary facilities, and conduct construction in ways and by methods that comply with environmental regulations, and minimize the possibility that air, waterways, and subsoil might be contaminated or polluted or that other undesirable effects might result. k. Restoration of Owner's existing facilities used as temporary facilities. B. HVAC Construction Contract (HVAC) 1. Work in the HVAC Construction Contract includes, but is not limited to the following: a. Provide new roof curbs. b. Provide new exhaust fans where required. c. Remove, store, and reinstall exhaust fans where required. d. Provide new strainer baskets for existing roof drains.
