To request services to remove hazardous trees on the Aiea Loop Trail at the Keaiwa Heiau State Recreational Area. SCOPE OF WORK: Contractor is responsible to furnish labor, materials & equipment necessary to perform the following in the specified State Park Area: Keaiwa Heiau State Recreational Area Contract Work as follows: See Attachment II Excel spread sheet for the tree work description and location. * All cut and mulched material may be left at the Keaiwa Heiau State Recreational Area. B. GENERAL NOTES: 1. For the purpose of this contract, Eric Kato, Oahu District Superintendent of the Department of Land and Natural Resources, or designee, is the designated Contract Administrator (CA). The telephone number at which he may be reached is (808) 265-2790. 2. All tree maintenance and removal operations shall be performed by a qualified arborist certified by the International Society of Arboriculture (ISA Certified Arborist) for a minimum of 5 years. a. For the duration of the trimming and removal there shall be one (1) qualified Arborist supervising the tree work. b. The Contractor shall provide the following, in writing, as proof of the qualified Arborist's current certification and qualifications: i. A copy of the named qualified Arborist's ISA (International Society of Arboriculture) certificate, which includes the expiration date, shall be submitted for approval by the CA, at the time of bid submittal. c. Possess (5) years of work experience as an ISA Certified Arborist which includes: i. Supervision of work similar in scope to the specified in this contract. ii. Knowledge and experience in the pruning practices and maintenance of all tree species included in this contract. iii. Experience in the recognition and diagnosis of problems and hazardous conditions affecting the tree species included in this contract. A sound knowledge and understanding of local tree species and their growth characteristics. iv. Knowledge and experience in canopy and root pruning practices for the tree species designated in this contract. v. Possess a thorough working knowledge of ANSI Z133 (safety standards), and ANSI A300 Best management Practices (pruning standard practices). vi. Demonstrated ability and experience to supervise the Contractor crew to ensure the satisfactory completion of operations including, but not limited to, insuring proper clearances, proper pruning and tree trimming techniques, proper tree removal techniques, productivity, traffic control, cleanup, safety, and compliance with this contract. vii. Ability to recognize and report hazardous conditions that cannot be addressed by corrective pruning described in this contract. Any contractor submitting a Certified Arborist Certificate without the minimum 5 year experience may not be considered by the CA. 3. The use of climbing irons, spikes, or other equipment that might injure the bark or cambium layer will be allowed upon consultation with the Certified Arborist and CA; except on trees that may be designated for removal. 4. When cut down and removal to low or no stump work is indicated, the remaining stump shall be treated with a growth inhibitor/herbicide such as Garlon_. 5. Free-fall of large tree materials is prohibited. Under no circumstances shall large limbs and branches be allowed to "fall with force". 6. Trimmings and debris shall not be left unattended at the work site by the Contractor, unless specified by the CA. All trimmings and debris shall be hauled away daily and disposed of by the Contractor as work progresses at a proper licensed waste facility, and leave the project area in a clean and neat condition. There shall be no deviation from requirements pertaining to removal of trimmings and debris unless specifically agreed to by the CA. 7. The Contractor shall be responsible for verifying the location of all utilities and structures. Any damage, injury, and/or loss to any property resulting from the performance of the work shall be the sole responsibility of the Contractor and shall be remedied promptly by the Contractor at his own expense. 8. The Contractor shall comply with all State Park rules when performing work in the specified park areas, including no alcohol consumption and illegal drug use. 9. The Contractor is responsible to perform their work in such a way as to minimize its impact on the operation of the park facilities. Prior to proceeding with the work, the Contractor shall contact, coordinate, and schedule the performance of the work with the Oahu District Superintendent. The work schedule shall be mutually agreed with the approval of the Superintendent. 10. All work must be done during normal park hours, unless prior approval is given by the CA or designee. The Contractor shall coordinate with the CA or designee, to determine the work schedule and access to parks. 2 11. The Contractor shall provide safety signs, barricades, and other devices necessary for the safety and convenience of the general public. Contractor shall at all times conduct his/her work to assure the least possible disruption to normal public traffic (vehicular and pedestrian), and allowing for the free flow of traffic and public rights-of way. 12. Any damage to plants and other property or items in the area and adjoining properties shall be repaired and/or replaced by the Contractor at his/her own expense and restored to the satisfaction of the Contract Administrator and the injured party(ies). 13. Adverse conditions which may require major field changes not stated in this contract must be reported to the Contract Administrator prior to commencement or continuation of work.
