Site work for a mixed-use development in Madrid, Iowa. Completed plans call for site work for a sidewalk / parking lot; and water / sewer project.
The intent of this project is to increase the length of the existing boat ramp by twenty feet by pushing the existing lower further into the river and pouring a new section in place. The work sequence is assumed to be the following: 1. When water levels are expected to remain below 857.5, the contractor is to place traffic control and break up and remove the grouted riprap that is located at the toe of the ramp. This material may be placed along the upstream side of the ramp. 2. Remove an approximately 2' long portion of the existing bottom slab, for the entire width, in order to allow enough room for a bulldozer blade to push on the remaining existing slab. The demolished concrete is to be hauled off site for disposal. 3. Grade the ramp subbase to match the existing 13.9% ramp grade, for a distance of 20 feet beyond the end of the ramp. Base rock consisting of 6" of Class A Crushed Stone is to be placed on the subbase. 4. Push the existing slab 20' into the river. 5. Pour new ramp slab in place. Tie into the existing slabs by drilling and epoxying in #4 rebar at 24" centers. Keep traffic control up for at least two weeks to allow concrete to cure. 6. Place Class E Revetment along any exposed edges of the ramp. Grout armoring in place. Located at: 1531 255th Street, Madrid, IA (Ledges State Park) in Boone County, Iowa The DEPARTMENT reserves the unconditional right not to award a contract for the project described in these instructions and related attachments. In the event the CONTRACTOR is required to be certified or licensed as a condition precedent to providing services, the revocation or loss of such license or certification will result in immediate termination of the contract effective as of the date on which the license or certification is no longer in effect Time is an essential element of the contract and it is important that the work be completed by the deadline. For each calendar day that any work shall remain uncompleted after the end of the contract period, number of working days allowed, or any extension granted, the amount per calendar day specified in the proposal form will be assessed, not as a penalty, but as predetermined and agreed liquidated damages. The CONTRACTOR hereby agrees that liquidated damages in the amount of $800.00 shall be retained or assessed against the CONTRACTOR for each day and every day the completion of the work is delayed beyond the time specified herein, not as a penalty, but as a mutually agreed to, predetermined amount to reimburse the DEPARTMENT for salaries and related expenses of directly involved staff and reviewers, interest charged during the period for delays and loss of use. In compliance with Chapter 573 of the Code of Iowa, contracts shall, when the contract price equals or exceeds twentyfive thousand dollars, be accompanied by a bond, with surety, conditioned for the faithful performance of the contract. The amount shall be not less than one hundred percent (100%) of the contract price. The bond shall be executed on the standard form supplied by the DEPARTMENT. The bond, if required, shall be executed by the CONTRACTOR and by a surety authorized to do business in the State of Iowa and shall be incorporated by this reference The time of commencement of work under this contract shall be within a reasonable period of time after the preconstruction conference as approved by the Department.
