Paving bid contract: Perform approximately 1473 SY of roadway surface milling, 22 CY of roadway shoulder milling/excavation, 156 CY roadway milling/ excavation, furnish and place approximately 168 tons 2A aggregate, 46 tons of 19.0 mm asphalt pave to reestablish shoulder, 205 tons of fine grade wearing course w/ 1/2" scratch course of 9.5 mm, 101 tons of asphalt binder course 19.0 mm 51 tons of wearing course, 9.5 mm; perform approximately 20 tons of 19.0 mm base repairs. Performance bond and payment bond in the amount of 100% of the contract, as well as proof of worker's compensation insurance will be required from the lowest responsible bidder. All work and material must meet the current Pennsylvania Department of Transportation Specification form 408 and its supplements. Proposals must be furnished upon the forms furnished by the Municipality. The Municipality reserves the right to reject any or all proposals. Pennsylvania prevailing wage rates shall be required for the paving bid contract only and are included with the paving contract. The Contractor is responsible to ensure that the proper material is provided by supplying: 1. A copy of an approved PENNDOT Asphalt Mix Design a minimum of 5 working days prior to the start of the work. 2. The Contractor will also supply the Municipality with a Daily Asphalt Material Certification on a TR-465 or CS-4171, supported by testing within 24 hours of placing the Asphalt material for each day's placement. 3.
