This Contract provides for the creation of in-channel and floodplain habitat on North Fork Newaukum River in Chehalis, WA (Project). This Work includes, but is not limited to: clearing and grubbing routes for construction of Project elements, installation of large woody material and engineered log jams including excavation and backfill, site isolation and de-watering in theriver, driveway culvert installation, site restoration, and other work in accordance with the attached Construction Drawings, these Contract Provisions, and the Washington State Department of Transportation - 2025 Standard Specifications for Road, Bridge, and Municipal Construction. This project includes the restoration of streambank along the North Fork of the New aukum River including: \b In-stream work area isolation, dewatering, and fish exclusion \b Meander bend ELJ, including 49 root wads & 35 driven timber piles, earthwork 800 cubic yards \b Installation of erosion control fabric associated to bank stabilization \b Install backwater channel \b Replace driveway culvert - Installation only This project is funded by grants administered by the Washington Recreation and Conservation Office. Washington State prevailing wage provisions apply to this project and contractors will be required to file a Statement of Intent to Pay Prevailing Wages to Department of Labor and Industries, as well as Affidavit of Wages Paid. The district will not make final payment until they receive evidence that the Affidavit form has been certified by L&I. All Bid Proposals shall be accompanied by a bid proposal deposit in the form of a Cash, Cashier's Check, Certified Check in an amount equal to five percent (5%) of such bid proposal. The Proposal Bond may be on DOT form 272-001 revised01/2020 for Local Agency Use. Should the successful bidder fail to enter into such contract and furnish satisfactory performance bond within the time stated in the Specifications, the bid proposal deposit shall be forfeited to Lewis Conservation District. All Bid Proposals shall be on forms from Lewis Conservation District. Paper bids shall be sealed and placed in an envelope, with the indication on the bottom left corner of said envelope, the project name, name of bidder, and date and time of opening, and then delivered to the Lewis Conservation District on or before the day and hour of the abovementioned bid opening. Lewis Conservation District reserves the right to accept or reject bids on each item separately or as a whole, to reject any and all bids, to waive informalities and to contract as to the best interest of the Lewis Conservation District.
