Demolition, site work and addition to a medical facility in Nokomis, Florida. Conceptual plans call for the addition of a medical facility; for the demolition of a medical facility; and for site work for a medical facility.
PROJECT DESCRIPTION: The scope of work for this project includes architectural and engineering services for the development of a 2-story expansion (the 2-story expansion) to the existing Sarasota Memorial Hospital-Venice (SMH-V) campus. Each level will be approximately 12,000 square feet, and the building will be approximately 24,000 square feet total. Level one will consist of a four-zone MRI suite, interventional radiology suite, prep recovery bays, procedure rooms, reading rooms and all associated circulation, life safety, and clinical support space. Level two will be shell space for a future expansion. The project is located at 2600 Laurel Road East, North Venice, FL 34275. The architectural and engineering services will include, but are not limited to, site planning, permitting documentation, City presentations, public meetings, community meetings, programming, schematic design, design development, construction documents, and construction administration for all planning, architectural, interior design, civil, site work, and site work infrastructure. The site work infrastructure also includes, but is not limited to, public utilities, communications providers, mechanical, electrical, plumbing, fire protection and structural design work for the development of the 2-story expansion, associated business services, and the required support services required for each, designed to allow for horizontal expansion capabilities. Furthermore, the 2-story expansion will include new surface parking to allow for the increase in pedestrian and vehicular volumes. The 2-story expansion also will include the relocation of an existing at-grade helipad. The scope of work may include consideration of additional buildings, roadways, and services whether contiguous to the hospital or freestanding, and the associated programming of those services. PROPOSED SCHEDULE: The timelines to design and build the above-described scope and services will be determined by the results of the planning study and potential revisions to the planning study as the 2-story expansion/campus development progresses. In addition, the proposed schedule will be determined, and may be subject to change, based on the timing of regulatory and other necessary approvals. Project development, including professional services, is contingent upon availability of funds. Firms interested in being considered as candidates are required to submit five (5) bound statements of qualifications, to be organized in the following order and including, at a minimum, the following information: 1. A copy of the firms current applicable Florida professional license(s) (e.g., Certificate of Authorization from the Florida Board of Architecture and Interior Design or the Florida Board of Professional Engineers) and corporate registration certificates. 2. Proof of insurance in the following categories: Commercial General Liability to include $1,000,000, each occurrence; Damage to Rented Premises, $300,000 each occurrence; Medical Expenses to any one person $10,000; Personal Injury $1,000,000; General Aggregate $2,000,000; Umbrella Liability $5,000,000, each occurrence; Worker Compensation & Employee Liability insurance minimum $1,000,000 each occurrence, $1,000,000 each accident, $1,000,000 disease each employee, $1,000,000 disease policy limit. The Firms insurer(s) shall be rated A- (or better) by A.M. Best. 3. A statement as to whether the firm is a certified Minority Business Enterprise (MBE) or Woman or Minority Owned Business Enterprise (WMBE). If your firm claims MBE/WMBE status, a copy of your Firms current, valid MBE/WMBE certificate is required as part of the submission package. 4. A list of the proposed design team to include individual resumes with relevant design experience as follows: a. Planning of projects similar in size and scope as described above in an active, occupied hospital building or campus; b. Programming of hospital projects similar in size and scope as described above in an active, occupied hospital campus, preferably in the state of Florida; and c. Examples of a similarly completed planning and design project that include: i. A multi-level expansion connected to a licensed acute care hospital and agency required support services, preferably in the state of Florida; d. Site development experience including, but not limited to: Roadways, Surface Parking, Utilities, Lighting, Landscaping and Drainage. 5. A list of at least five client references consisting of name, title, organization, address, telephone number and project name(s) for each reference specified. Please ensure your references are updated and willing to reply. Reference responses are mandatory. 6. A summary of the firms design and permitting experience within the City of Venice, Florida and other applicable permitting agencies. 7. The location of the design firms main office, and proposed project team office location (if different from main). 8. An explanation of how the firm will respond expeditiously to urgent project matters, and a summary of the firms proposed approach to this particular project. The basis for selection criteria for this project includes, but is not limited to: 1. The firms relevant design experience as it relates to the above referenced project; 2. The firms relevant experience with regulatory agencies with applicable jurisdictional authority, including, but not limited to, federal, state, and local agencies; 3. The firms depth of design team and the resources available for this assignment. This includes recent, current and projected workloads of the firm; 4. The location of the lead designers main/corporate office; 5. The firms approach to this particular project; 6. The firms ability to respond quickly; 7. Whether or not the lead firm is an MBE/WMBE (valid certificate from the Office of Supplier Diversity required); and 8. The volume of previously awarded projects to the firm by the Hospital. The Selection Committee may use a scoring rubric or consensus method to evaluate qualifications based on the criteria described above. No price or fee proposals shall be considered during the evaluation phase, in accordance with Section 287.055, Florida Statutes. It is the Hospitals responsibility to negotiate a fair, competitive and reasonable compensation per Section 287.055, Florida Statutes. A fair, competitive and reasonable compensation shall be evaluated based upon the following information: (1) Compensation on similar projects; (2) other compensation reference data; and (3) after approval of the ranking, proposals requested from the shortlisted firms based upon a scope of services document to be provided at the time of negotiations. All interested Firms are further informed as follows: 1. The Hospital reserves the right to reject any or all submittals at any time during this process. 2. The Hospital reserves the right to request additional information beyond the data set forth above. 3. The Hospital reserves the right to waive any minor irregularities, informalities, or technical deficiencies in a submittal, provided that such waiver does not materially affect the qualifications of the responding firm. 4.
