BASE BID - $415,000.00 the following roadway resurfacing project: Hoose Road Resurfacing. COMPLETION DATE: 10/31/2025 Scope of repairs for inlet basins reconstructed _ Saw cut and remove material from each basin's "Box Out" area. _ Remove all basin structure down completely or to a solid course. _ Notify City of Mentor if a "casting" is bad. The City of Mentor will provide a replacement casting. Rebuild basin up to required level. Note: To prevent standing water, a positive flow must be established from the top of the existing concrete perimeter to the top of the grate/castings final grade. Also, the contractor must use "Hard Red" sewer or manhole brick, ASTM C-32, grade M.S. Haydite / Concrete brick will not be permitted. _ Apply a parge coat, Type "S", to the entire exposed inside surface before setting the casting. _ Set casting and form the area. _ Remove undesirable, soft sub-grade soil. _ Drill and install 1" diameter epoxy coated dowel pins in accordance with the basin detail provided. _ Install #6 "Road Mesh" in "Box out" area. _ Install rubber expansion joint (fiber will not be permitted), along all sidewalk or drive apron areas only before inspection and pouring. _ Contact the City of Mentor Public Works Department (440) 974-5781 for inspection prior to pouring medium set (MS) concrete at a minimum depth of 8 inches. _ Apply Concrete Sealer: ChemMasters "Silencure DOT" (or equal) Concrete Sealer: Apply after all bleed water has dissipated and application will not mar the surface. Follow all manufacturers recommended processes, precautions, and safety procedures. _ After 24 hours, saw cut relief joints in new concrete, blow out debris and then crack seal all joints with hot applied Crafco 34515 modified sealant (ODOT Type IV) or equal.
