Renovation of an elderly care / assisted living facility in Ogdensburg, New York. Completed plans call for the renovation of a elderly care / assisted living facility.
All bids must be written in ink on the forms provided. Bids must be accompanied by bid security equal to five percent (5%) of the amount of the bid, in the form of a certified check, drawn upon a national or state bank or trust company to the order of "City of Ogdensburg Housing Authority", or a bond from a surety company licensed in the state of New York, with sufficient sureties in a penal sum, and holding certificates of authority in compliance with 31 CFR part 223 Surety Companies Doing Business with the United States., conditioned that if Bidder's proposal is accepted, bidder will enter into a contract for the same, and will execute separate Performance Bond and Labor and Materials Bond each equal to one hundred percent (100%) of the Contract Amount. Bid Bonds and Performance Bond and Labor and Materials Bond shall be obtained from Surety Companies with a Best Rating of A (excellent or better) and holding certificates of authority in compliance with 31 CFR part 223 Surety Companies Doing Business with the United States. Bids must be accompanied by a completed Statement of Surety's Intent. If the selected Bidder fails to sign the Contract and deliver the required bonds, he shall be subject to forfeiture of the bid security. If Bidder's proposal is accepted, bidder will enter into a contract for the same, and will execute separate Payment and Performance Bond each equal to one hundred percent (100%) of the Contract Amount. Payment and Performance Bond shall be obtained from Surety Companies with a Best Rating of A (excellent or better). Owner may release addendums up to two days prior to the bid due date, with the exception of an addendum changing the bid due date.) Event Bid notes: Single lump sum bids are required accordance with plans and specifications. Sealed bids shall be marked 'Ogdensburg Housing - Boiler Replacement - Parkview Rise.' Emailed bids are also acceptable and must be sent to cbasford@ogdensburgha.com, athomas@labellapc.com and tcassell@labellapc.com. The successful bidder will be required to comply with all applicable federal, state and local laws and regulations. The Ogdensburg Housing Authority (OHA) follows all rules and regulations set forth by the United States Department of Housing & Urban Development (HUD). Ogdensburg Housing Authority is an Equal Opportunity employer and encourages MWBE to participate in the Bid process. Bidders will provide proof of soliciting MWBE business or provide form MWBE 101- Wavier Request. Attention of bidders is particularly called to the requirements as to conditions of employment to be observed. This project is subject to Davis Bacon Wage Rates. Ogdensburg Housing Authority reserves the right to consider all bids for a period of not to exceed forty-five (45) days following the bid opening before awarding the Contract, during which period bids shall not be withdrawn, and reserves the right to reject any or all bids, to waive any informality in the bids, and to award the Contract in the Owner's best interest. The date of substantial completion for the work shall be no later than 110 days after contract award. A penalty of one hundred dollars ($100.00) per day may be assessed for late substantial completion. All bids must be written in ink on the forms provided. Bids must be accompanied by New York State VendRep System, Vendor Responsibility NFP Form. A list of any subcontractors and solicitations of Minority and Women-Owned Business Enterprises (M/WBE). Bids shall be accompanied by a completed Non-Collusion Statement as provided in the Project Manual. Certified Minority and Women-Owned Business Enterprises (M/WBE) and Section 3-elgible contracting firms are encouraged to consider this Project.)
