The project focuses on creating a more inclusive and comprehensive patient care atmosphere, allowing for more integrated care by fostering linkages from ambulatory care to inpatient care and pediatrics. Further linkages with the maternal home team and social workers will provide support services to patients in the community. Health + Hospitals seeks a comprehensive vision of Integrated Women's Health services in New York City Health + Hospitals and to provide safe, respectful care to women throughout the childbearing cycle. PROJECT SCOPE 1. Renovate approximately 17,000 sq. ft of Integrated Women's health services on one floor 2. 28 large exam rooms that will also accommodate enough space for strollers and designed so patients can receive care in the same room without having to return to the waiting room or move to another department. 3. Bariatric friendly Exam and Treatment rooms 4. 6 Ultrasound Rooms 5. 6 Multipurpose rooms for procedures to include but not limited to Urodynamics, Colposcopy and Hysteroscopy. 6. On hand staff support office and touchdown space. 7. 2 Large education rooms for staff and patient use. 8. Larger and more comfortable waiting areas for patients and their family members have liquidated damages assessed commensurate with NYC Health + Hospitals' efforts that result from Design-Build Contractor's non-compliance, bad faith or breach of this section, which shall in no event be less than $5,000.00, or the amount represented by the difference between the subcontracting goal dollar value set forth in Design Builder's Utilization Plan and the amount actually paid by Design-Builder to Diverse Contractors. NYC Health + Hospitals reserves the right in its sole discretion to (i) disqualify any Respondent that changes its SOQ without NYC Health + Hospitals' approval, (ii) hold meetings and exchange correspondence with the Respondents responding or intended to respond to this RFQ to seek an improved understanding and evaluation of the SOQs, and (iii) disqualify any Respondent whose conduct/and or SOQ fails to conform to the requirements of this RFQ.
