Sealed FILED SUB-BIDS for categories of Historic Masonry, Waterproofing Damproofing and Caulking, Roofing, Painting and Electrical A 5% Bid Bond or Certified Check must accompany each bid submitted and made payable to, and become the property of the City of Waltham, if the successful bidder refuses or neglects to comply with the terms of the Contract. In accordance with M.G.L.Ch 149 the undersigned certifies that all employees to be employed at the worksite will have successfully completed a course in construction safety and health approved by OSHA that is at least 10 hours in duration at the time the employee begins work and shall furnish documentation of successful completion ofsaid course with the first certified payroll report for each employee.
