Pursuant to Texas Government Code 2252.908, on behalf of Fort Bend County Municipal Utility District No. 222 (the "District") with a printed, executed and notarized original of a completed Certificate of Interested Parties form (Form 1295) with the bid. Failure to do so will result in the District's inability to execute the contract.
