Reference Number 0000398044 The City of Madison Heights (hereinafter City) is soliciting proposals from qualified contractors to replace the aged and failing roof and tongue-and-groove decking at Pavilion C, located in Civic Center Park. The general scope of work is to remove and replace approximately 1,750 SF of existing shingle roofing and replace rotted tongue-and-groove decking. Work must be completed within 60 days of Notice to Proceed (weather permitting). One original and three (3) copies of the proposal shall be submitted in sealed envelopes and shall include the following information on the face of the envelope: Contractor's Name Address RFP Number and Item Failure to do so may result in a premature opening of, or failure to open, such proposal. Buyer Preferences, Guidelines & Requirements General Requirements - Installation Required - Insurance Required - License Required - Warranty Information Required The City reserves the right to waive any informality in the proposals received and to accept any proposal or part thereof, which it shall deem to be most favorable to the interests of the City of Madison Heights. General: The City of Madison Heights (hereinafter City) is soliciting proposals from qualified contractors to replace the aged and failing roof and tongue-and-groove decking at Pavilion C, located in Civic Center Park. The general scope of work is to remove and replace approximately 1,750 SF of existing shingle roofing and replace rotted tongue-and-groove decking. All measurements are to be verified in the field by the contractor. Alternate pricing (Alternate 1) for the removal of the tongueand-groove decking, without replacement, must be included. Disposal of all materials is to be included in the proposal. A similar metal roof can be observed on the pavilion at Monroe Park (479 W. Dallas, Madison Heights). Scope of Work: The proposal shall provide for all labor, materials, tools, and equipment as required to provide the following: (1) Equipment: a. Approximately 1,750 SF of metal roofing (roofing materials, screws, etc.); measurements to be verified by contractor in the field. b. Replacement of rotted existing tongue-and-groove decking c. Installation of metal roof support(s) if the tongue-and-groove decking is fully removed. d. The metal roof shall be forest green in color with hidden fasteners preferred but not required. (2) Project Oversight: a. Project Manager: The contractor will provide one (1) person on-site as the project manager and primary point of contact. This project manager will handle all project coordination with the City's point of contact, as well as any issues that may arise during the project. Further, the Project Manager will complete the final site visit and inspection with City personnel. (3) Construction Process: a. The Contractor shall be responsible for all signage, barricades, or other devices to provide for a safe and visible construction zone while work is being performed. At no time shall an open hole or work area be left unattended. b. Any ruts or settlement created by construction equipment shall be filled to the surrounding grade with topsoil, with grass seed raked in to restore the turf. c. The Bidder is responsible for maintaining the site during and after the work. Any damage to the City building or property shall be the Bidder's sole responsibility. (4) Project Completion: a. Work must be completed within 60 days of Notice to Proceed (weather permitting).
