The objective of this RFP is to select the most competitive and qualified vendor to provide bridge decking for the Crutcher Pike bridge. Mandatory Proposal Requirements: Interested vendors are requested to submit the following formation as part of their proposal: Material List Deck Forms 14 pcs 19'-3" (45" cov) Outside Angles 10 pcs 2" x 6" x 10'-0" End Angles 4 pcs 2" x3" 12GA 12'-0" Flat straps for braces 8 pcs. 2" x 12'-0" o Material: Material conforms to ASTM A653 with a coating class of G-165. Grade 80 Steel is used for deck gauges 18 through 22, providing 60% more stress capacity than Grade 50, and superior resistance to construction abuse. o Form Sheets & Accessories: Closed-end form sheets with gauge to match requirements per contract drawings. Inclusions: For main slab: bridge form pans, support angles, closures, screws, and shop drawings. Materials quoted herein allow for a maximum haunch dimension at edge of girder flange of 5". o Delivery to Madison County Road Department at 325 N. Madison Avenue, Richmond, KY 40475 o Please provide lead time for the decking materials. 3. Facsimile and E-mail Responses: Proposals will not be accepted by facsimile or email. 4. Corrections, Modifications, or Withdrawal of Proposals: The County reserves the right to reject any or all proposals. After opening of the proposal responses, a Respondent may not correct, modify, or withdraw the price or any other provision of its response in a manner prejudicial to the interests of the County or fair competition without prior written approval from the County. The County may waive minor informalities or allow the Respondent to correct them. 5. Rights to Proposal, Bid, Quotation, and Contractual Material: All material submitted by the Contractor to the County shall become property of the County upon receipt. 6. Invoicing: Remit invoices to: Madison County Road Department 325 N. Madison Avenue Richmond, KY 40475 ATTN: Melissa Settle, Office Manager OR Email to: melissa.settle@madisoncountyky.gov Unless otherwise instructed in writing, the County may rely on the remittance address specified on the Contractor's invoice. Each invoice shall be written in sufficient detail so that the invoice may be reviewed, evaluated, processed, and paid without the need to contact the Contractor to obtain additional information. Detailed information to be contained in each invoice shall be specified by the County following contract award. Any invoice for any County-approved work activity not proposed, contracted, and included in the SOW must include a written and pre-dated County authorization and be detailed separately. 7. Occupational Licenses: All Bidders must have or obtain a Madison County and City of Richmond's Occupational License prior to performing any work in Madison County per Ordinance 06-18 or in the city of Richmond, KY, Madison County. Bidders must be in full compliance with all Madison County's Occupational License Ordinance 06-18, and the City of Richmond Occupational License Ordinance 07-40 prior to the awarding of a contract. Madison County shall have the right to reject any bids/offers from Bidders that are not in compliance with Madison County and City of Richmond's Occupational License Ordinances. 8. Inspection: Materials or equipment purchased are subject to inspection and approval at Madison County's destination. Madison County reserves the right to reject and refuse acceptance of items which are not in accordance with the instructions, specifications, drawings or data of Seller's warranty (express or implied). Rejected materials or equipment shall be removed by, or at the expense of, the Seller promptly after rejection. 9. Equal Employment Opportunity: Contractor shall comply with the Kentucky Human Rights Act, HRA 344.150 - 344.270 as amended and any rules and regulations promulgated in accordance therewith, including, but not limited to the Equal Employment Opportunity EEO 45.550 - 45.640 which is incorporated herein by reference. Furthermore, the Contractor shall comply with the Employment Discrimination Act, EDA 344.040, 344.050 as amended. 10. Liability Insurance: Unless otherwise provided herein, Liability Insurance in the amount of $1,000,000.00 must be maintained throughout the life of the Contract. A Certificate of Insurance naming Madison County as co-insured must be submitted prior to execution of the Contract. The policy must provide that Madison County (as coinsured) shall be given at least a 10-day advanced written notice of any proposed cancellation or termination of the policy. 11.Workers' Compensation: A Workers'Compensation certificate shall accompany all bidding documents as proof of payment. Madison County may request proof of Certification at such other times during the life of the Contract as it deems necessary. 12. Indemnity: The Contractor shall indemnify and hold harmless the County, including its 5 officers, employees and agents, from any and all liabilities, claims, debts, damages, demands, or actions of whatever kind, nature, or sort (including, but not by way of limitation, wrongful death, bodily injury, property damage, and legal expenses) arising out of or in any manner connected with the Contractor and their approved subcontractor's performance related to the Contract.
