An Executed Bid Bond Shall Be Included With All Bids. Only Electronic Bid Bonds Submitted Through the Designated Electronic Medium, Either Surety2000 or Tinubu Surety Solution, Will Be Accepted With the Bid. A Fully Executed Performance Bond Shall Be Included With All Bids, Except on Contracts for Roadside Mowing, Vegetation Control, Demolition, Landscaping, Fencing, Seeding and Sodding, Bridge Painting, and Guardrail Installations of $100,000 or Less, When the Bidder May Elect Not to Furnish a Performance Bond. The Performance Bond Must Be Submitted to the Construction Letting Email Inbox or Be Delivered on Paper by the Time Set for Receipt of Proposal Bids. Each Contractor That Submits a Proposal Bid in the Amount of $150,000 or More for Highway Improvements Described in This Notice is Required to Submit With That Bid a Written Plan for a Program to Test the Contractor's Employees for Drugs. The Plan Shall Be in Compliance With Indiana Code 4-13-18. The Plan Must Be Submitted to the Construction Letting Email Inbox or Deposited With the Indot Engineer by the Time Set for Receipt of Proposal Bids. INDOT - Call 400 - Bridge Deck Overlay, Small Structures and Drains Construction - Multiple Locations This work shall consist of furnishing an electronic material delivery ticket, e-ticket, and delivering the e-ticket to the Department's e-ticketing portal, MyDOTportal, in accordance with 105.03. This work shall consist of digging inspection holes in accordance with 105.03, to verify the exact location of underground utilities that are in potential conflict with the proposed construction. Recycled foundry sand, RFS, consists of a mixture of residual materials used from ferrous or non-ferrous metal castings and natural sands. The Contractor shall have the option of incorporating RFS into applicable operations in accordance with 105.03. This work shall consist of furnishing and placing elasticized expanded polystyrene, EPS, and geotextile fabric between the back of concrete surface and backfill material in accordance with 105.03, the manufacturer's recommendations, and as shown on the plans. INDOT Des. 2100749 is a bridge preventative maintenance project on US 6 over Salt Creek (006-64- 06697). The structure is a continuous composite steel beam bridge with a total length of 201'-10 5/8", built in 1985 and reconstructed in 2008. The proposed work includes placing a rigid overlay, converting to semi-integral end bents, placing new reinforced concrete bridge approach slabs, placing new pavement to tie into the new bridgework, installing buried drain pipes behind the new end bents which will outlet away from the roadway, placing revetment riprap along bare areas of the spillslopes, and upgrading the existing guardrails to MASH preferred MGS W-beam guardrail system. Revetment riprap was placed for scour protection at this bridge in 2008. The new 18" revetment riprap proposed for this project will be placed in areas of the spillslope where the existing riprap has thinned or is bare. The riprap will not be placed below the Ordinary High Water Mark (OHWM). Estimated quantities are approximately 10 Tons or an area equivalent to 10ft by 12ft. The proposed project consists of bridge preventive maintenance on US 6 over Salt Creek, 0.15 mile east of State Route (SR) 149 in South Haven, Liberty Township, Porter County, Indiana. Existing US 6 is an urban principal artillery road with two 12-foot concrete travel lanes, one in each direction, bordered by 9-foot, 9-inch wide, concrete shoulders on either side. The existing bridge, Bridge No. 006-64-06697A/ NBI: 001620, consists of a 78.5-foot span concrete cast-in-place, with a total length of 202.1 feet, built in 1985 and reconstructed in 2008 and as such, does not meet the minimum 50-year age requirement for National Register evaluation. The project scope of work will involve placing a bridge deck overlay, converting the end bents to semi-integral, placement of new reinforced concrete bridge approach slabs, riprap placement to fill in missing riprap, and replacing the existing guardrail. No permanent or temporary right-of-way will be required and no tree clearing will occur. No impact will occur to the wildlife crossing beneath the east and west ends of the bridge. The riprap will be placed in areas with existing fallen riprap and will not extend into the wildlife crossing. A provision will be added that contractors will not place riprap in the wildlife crossing. Topographic maps and an aerial map showing the project location can be found in Figures 1-4. The investigated area was selected because it encompasses the area where work is anticipated to be completed for the project. The general topography of the investigated area is level with riparian forest. Residential properties are in the northwest quadrant, and commercial properties are in the southwest quadrant.
