Site work, new construction and renovation of a municipal facility in Carrollton, Kentucky. Completed plans call for the construction of a municipal facility; for the renovation of a municipal facility; and for site work for a municipal facility.
The City of Carrollton is seeking sealed bids from qualified bidders for fiscal year beginning July 1, 2025, and ending June 30, 2026, for the construction of a new Restroom / Entertainment Pavilion. Project will be a Design-Build New Construction project. Bidders will be provided with Architectural and Structural Drawings. It will be the responsibility of the bidders to provide Civil, Mechanical, Electrical and Plumbing Engineered drawings if required by the State of Kentucky. The City of Carrollton will not discriminate against any bidder on the grounds of race, color, national origin, sex, age or disability. Bidders will be required to be bonded for this project. Bidders shall provide a price for the entire scope of design and construction based on Architectural and Structural drawings plus any State required additional engineering. The Bidder shall be responsible for compliance with all building codes, guidelines, plan reviews, and construction inspections of all agencies having jurisdiction. Required insurance: Workers Compensation Prior to the commencement of work, the design-builder agrees to provide proof of Worker's Compensation insurance for his/herself, and all authorized employees and/or agents working under the executed agreement in an amount at least equal to $1,000,000.00 naming the City of Carrollton as additionally insured. General Liability: The Bidder agrees to have and maintain general liability insurance in the amount at least equal to $1,000,000.00 per occurrence naming the City of Carrollton as additionally insured. Indemnification: The Bidder shall indemnify, defend, and hold harmless the City, its agents and employees from and against all claims, damages, losses, and expenses, including attorney's fees arising from the execution of the work under this contract. Time of completion: The entire scope of the project shall be completed within (240) calendar days from the bid award date. Liquidated damages in the amount of $200 dollars may be assessed each day the project exceeds the scheduled date of substantial completion. Substantial completion is defined as the date of the Certificate of Occupancy issued by the agencies having jurisdiction. o The owner and their representatives shall perform a "Punch List" inspection of the completed work. Once all punch list items are completed to the owner's satisfaction, a letter of final acceptance shall be provided to the Bidder. Payment procedures: Schedule/Schedule of Values: Upon bid award, the design-builder shall prepare a project schedule based on the phases of work as defined herein. The Bidder shall also prepare a schedule of values coordinated with the project schedule. The schedule of values shall depict the anticipated percentage of completion throughout the distinct phases of the project. The schedule/schedule of values will be utilized to monitor progress and to verify the validity of periodic requests for partial payment. Project Contingency: The Bidder shall include in their bid a contingency line item of 8% of their total bid. The contingence shall be listed as a separate line item on their schedule of values. The line item will be used to cover unforeseen items that may be encountered throughout construction. However. The Bidder may only bill against the line item with mutual consent of the owner in the form of an executed change order. Any unused portion of the contingency will be credited back to the owner at the conclusion of the contract. Periodic request for partial payment: Requests for partial payments shall be made monthly throughout the duration of the project. Pay requests will be reviewed based on the Bidders schedule/schedule of values and per the materials installed and work performed on site. Payment will be processed within 30 days of the owner's approval of requests. Release of liens: Bidder shall attach a "release of liens" with each request for partial payment verifying that there are no unpaid mechanics or material liens. Retainage: 10% retainage is to be withheld from each payment and shall be accumulated on the schedule of values. The retainage shall be held until the owner's issuance of a certificate of substantial completion and the building departments certificate of occupancy, Retainage may be reduced to 5% after 50% of the construction has been achieved with the owner's approval provided that no unresolved issues exist. The owner will release retainage within 30 days of verifying that all punch list items are corrected, all issues are resolved, and final releases of liens have been submitted. Change procedures: Written directive: Changes not affecting the schedule or contract sum or in a case where there is a lack of agreement to the adjustment of the contract related to the change, the owner may issue a written directive to proceed with the work. The Bidder shall commence the change immediately upon receipt of the directive. Change Order: Changes requiring a modification to the schedule and/or the contract sum shall be issued by a change order. Executed change orders require the signature of the Bidder and the owner. No work shall proceed on the change until the order is fully executed. The City reserves the right to accept or reject any or all bids. MELINDA WRIG This project consists of a new restroom and multi fuction pavilion structure. The Restroom portion is 744 square feet. The covered Pavilion is 1,240 square feet.
