Demolition for a clubhouse / community center in Douglasville, Georgia. Completed plans call for the demolition of a clubhouse / community center.
The project consists of the demolition and removal of a 4,500 square foot building located at 8595 Club Drive, Douglasville, GA 30134. Competitive sealed bids are requested for this work. Insurance and onsite maintenance are required, as well as performance and payment bonds (100% each). Samples must be provided as part of the bid. Bids must be submitted electronically, and the pricing should be in lump sum format. Scope of Work 1. Demolition and removal of 8595 Club Drive building 2. Contractor responsible for removal and disposal of all materials including transport of materials and all related permits in accordance with local, state, and federal law. 3. All utilities have been disconnected by the City. 4. Hazardous Materials Survey: Exhibit A B. Format for Submission of Bid Proposal Bid Proposals shall include the following elements: 1. Firm name, address, and telephone number 2. Primary contact person(s) and telephone number(s) 3. Year firm was established. 4. Provide at least 3 client references and contact information. 5. Cost of proposal 6. Organizational Chart which identifies individuals names and responsibility 7. Proposed timeline for project. 8. Brief statement of Qualifications 9. Contractors and Subcontractors affidavits of compliance with the Georgia Security and Immigration Act. (E-Verify) (on form supplied in RFB)
