The project includes, but is not limited to, the replacement of broken brick sidewalk on E Kirkwood Ave from N Indiana Ave to N Walnut St. Bids are to be submitted in proper form as described in the instructions. Each bidder must include with their sealed bid: a properly executed Questionnaire Form 96 of the State Board of Accounts; a cashier's check or certified check, or a bid bond equal to five percent of the total bid amount; a properly executed Trench Safety Systems Affidavit if the project requires a trench of at least five feet; a copy of the bidder's written plan for an employee drug testing program if the estimated project cost is $150,000 or more; and a signed contract compliance certification form. For projects estimated to cost at least $200,000, wage rates must comply with the Davis Bacon Act, and both a performance bond and a payment bond for 100% of the contract amount must be furnished prior to contract execution, remaining effective for one year after final acceptance of the work. For bids over $300,000, additional pre-qualification with the appropriate boards is required. Contractors considered covered employers must pay covered employees at least a living wage per local ordinance and may count a portion of health insurance contributions toward this wage. Bidders must also comply with nondiscrimination, anti-harassment, grievance, and anti-retaliation policies. Opportunities for training and employment should be given to lower-income residents of the project area, and contracts should prioritize small locally owned businesses where feasible. Failure to provide all required documents and forms may subject bidders to disqualification. The Board of Public Works reserves the right to waive any informality and to accept or reject any or all bids submitted.
