This project includes, but is not limited to, the installation of approximately 650 feet of sidewalk along the east side of Jefferson Street between 8th and 10th street. Other improvements involve installation of curbs, storm inlets, and drive aprons. Each bidder must submit: 1. A properly executed Questionnaire Form 96 of the State Board of Accounts; 2. A cashier's check or certified check from an acceptable bank or a Bid bond equal to five percent of the total bid amount (if the project is estimated to cost at least $200,000); 3. A properly executed Trench Safety Systems Affidavit, if the project may require the creation of a trench at least five feet in depth; 4. A written plan for an employee drug-testing program, if the estimated cost of the project is $150,000 or more; 5. A signed contract compliance certification form. For federally funded projects, wage rates must comply with the Davis Bacon Act. For bids of $200,000 or more, the successful bidder must provide both a performance bond and a payment bond for one hundred percent of the contract amount, which remain in effect for one year after final acceptance of the work. All bidders must ensure to the greatest extent feasible that opportunities for training and employment are given to lower-income residents of the project area, and contracts are awarded to small businesses located in or owned by residents of the project area."
