Scope of Work: - Building Permit fees are not applicable, but awarded firm must apply for a Permit. - Project is Tax Exempt. - Davis Bacon Act Wage Scales are applicable; current rates included. - Bidder must have a current State of CT Dept. of Consumer Protection License. - Insurance Certificate (with Town of Lebanon as Certificate Holder) must be submitted with bid: a. General Liability: $1,000,000 per occurrence b. Automobile Liability: $1,000,000 per occurrence c. Workers Compensation: Per State Standards - Town retains right to award contract to the lowest qualified, responsible firm. - Town of Lebanon is an Equal Opportunity Employer. - Submission of Monthly Wage Scale Certificates of Tradesmen required. - Furnish and install all window blocking (14 units) per Detail 14/A-9.1 - Install all windows as furnished by the Owner (14 units). - Furnish and install Azek trim at new windows per Detail 14/A-9.1 - Wood blocking at all exposed 8" foundation wall per Detail 1/A-4.1 - Detailed shop drawing submission required for review and approval prior to fabrication. - AIA Lump Sum Contract Agreement with Owner and Contractor, with Construction Manager as Advisor. Instructions to Bidders: - Submit all questions by 12/15/25 to Jim Russo/J R Russo, LLC. Responses will be posted on Town website. - Submit all proposals to First Selectman's Office at Lebanon Town Hall in a sealed envelope by 1:00 pm 12/19/25. Public opening will follow. - All bid packages must include: o Total Lump Sum Bid Quotation o Copies of all applicable State of CT Dept. of Consumer Protection licenses o Current Certificate of Insurance o Three professional references of similar projects o Signed submission of Suspension and Debarment Statement. - Project completion by March 13, 2026.
