Renovation of a medical facility in Morehead City, North Carolina. Completed plans call for the renovation of a medical facility.
he Contractor shall abide by the details and shall refer to the associated diagrams in addition to the scope of work specified within the documents provided below. General Scope of Work o The Contractor shall remove and dispose the identified existing flooring and cove base highlighted in Yellow on the diagram below. o Furnish and install Philadelphia Commercial LVT, Collection: Tecton, Style: Serra 00715, thirty-six-inch by thirty-sixinch (36" x 36") heavy commercial luxury vinyl tiles, (or equivalent approved by the County) installed using the Ashlar method in accordance with the manufacture's recommendations and instructions. o Furnish and install new four-inch (4") black toed cove base in all areas receiving new flooring. o Furnish and install any necessary thresholds, with the color of the thresholds matching the cove base. o Any new flooring that needs to be cut shall be precise, leaving no gaps. o All preparation of the existing subflooring required for the installation shall be included and completed by the Contractor. o The Contractor shall be responsible for moving all furniture and equipment, which shall be moved back in its original place before start of business. o Removal and installation of commodes shall be the Contractor's responsibility. The Contractor shall furnish and install new wax rings and hoses as required. leaning and Miscellaneous o The Contractor shall provide all necessary labor, materials, Safety Data Sheets (SDS), equipment, and a competent supervisor to oversee and direct work as outlined in these documents. o Protect all surfaces, furniture and fixtures from dust and debris. All associated debris shall be cleaned and hauled off at the end of each working day. All cleaning equipment shall be provided by the Contractor. o Any damages caused by the Contractor shall be the Contractor's responsibility to fix or replace at the County's discretion. o All measurements and material quantities shall be verified by the Contractor. o All new materials are subject to approval by the County. If the Contractor proposes using material equivalent to those specified above, detailed submittals shall be included with the Contractor's proposal for review and approval. o The awarded Contractor shall enter into a Contract with the County, adhering to the Terms and Conditions and Agreement for Services therein. o The Contractor shall obtain the required permits, if required, give all notices to the County POC (including a copy of any permits), and comply with all laws, ordinances, codes, rules and regulations bearing on the conduct of the work. Permits shall be posted onsite at project location. No time extensions shall be granted or financial consideration given to the Contractor for time or monies lost due to violations of any such ordinance, code, law or regulations that may occur. Alternate Bid No. 1: The Contractor shall provide a separate cost to replace the flooring in the areas highlighted in Red on the diagram below while following the base bid instructions. Alternate Bid No. 2: The Contractor shall provide a separate cost to replace the flooring in the areas highlighted in Orange on the diagram below while following the base bid instructions. Carteret County Government reserves the right to award to multiple contractors deemed to be fully qualified and best suited among others submitting proposals on the basis of the evaluation factors included within this RFP. Once the proposals are ranked by Carteret County Governments Committee and the qualified contractor is determined, Carteret County Government may conduct further negotiations, and/or request presentations to any or all contractors. Carteret County Government reserves the right to accept or reject, in whole or in part, proposals directed to the best interest of the County. Each contractor shall fill out the required information within this RFP, submit Certificate of Insurance for liability and Worker's Compensation in order for the proposal to be qualified. Carteret County Government reserves the right to reject any or all proposals for any reason.
