Bidders shall submit one original and two copies of the bid form with all specification pages. All pages of the Invitation for Bid/Request for Proposal must be used when submitting the bid/proposal response with initials on each page. No facsimile or electronic bids will be accepted. Late bids will not be accepted. A fully executed Affidavit as required by Section 285.530 RSMo must be submitted with the bid form. The bid may be awarded on an item-by-item basis or by total to the lowest and best Bidder(s) based on value to the County, including suitability, quality, service, experience, price, and delivery ability. The County may reject any or all bids and may waive any informality. The successful Bidder shall maintain required insurance at their own expense, including comprehensive general liability insurance ($1,000,000 per occurrence), professional liability insurance ($1,000,000), and worker’s compensation insurance per state statute. Jefferson County must be added as an additional insured. Required documents include Certificate of Insurance or binder, proof of no delinquent property taxes, affidavits of work authorization and business entity status, agreement to be executed by the County, Cooperative Bid Form, and all initialed pages. Bid deposits or bonds must be in the specified amount if required. Documentation omissions, failure to strictly comply with terms, or missing insurance may result in bid rejection. Alternate bids are allowed unless "NO SUBSTITUTIONS" is specified, in which case complete specifications must be submitted. Any addenda are considered incorporated. All conditions and forms are to be examined thoroughly before submission. Failure to comply may result in disqualification.
