This event is to bid for a one-time purchase to repair bituminous asphalt with asphaltic concrete. Specifications and terms & conditions are attached. Intersection repair: Construct a concrete slab (Approximately 5220 SQ FT surface area) at the Milan VTC in Lavinia TN. Suppliers are encouraged to attend a site visit to obtain the exact measurements, as the State will not be responsible for incorrect measurements or be responsible for any additional cost due to suppliers incorrect measurements This project is a four-way intersection that must maintain a design for heavy tactical vehicles (Tracked vehicles up to 70-ton rating) The replacement shall be constructed of a concrete slab over a 30-inch culvert and a six-inch force main sewer line on the south end. The slab will have a foundation around the complete parameter of the slab. Contractor shall ensure the foundation will be 24 -inches in width and extend 8-inches below the bottom of the main slab. The foundation will have #4 rebar horizontally positioned and tied to a vertically positioned #4 rebar every 12-inches and will extend up to stop within 2-inches from the top of the 8-inch slab. The pad must extend at least 3 feet beyond the forced sewer main, 8-inch water line, and fiber communications in all applicable directions. General specifications pertaining to the proposed replacement slab. The existing road that the slab will be constructed across is concrete. The debris from cutting the road can be dumped at a designated location on the facility. The concrete slab will have a 4- inch compacted sub-base of #57 crushed stone. The concrete mix will be 5000 psi with fiber. Contractor shall provide a pump truck to create one continuous pour to ensure no expansion joints. Contractor shall ensure the slab is 8 inches thick. Contractor shall ensure this slab will be reinforced using #4 tied rebar placed using rebar chairs that position the rebar approximately 4 inches above the compacted subgrade. The rebar will be placed at 12 inches on center in both directions and will terminate within two inches from the slab edge unless it can be drilled and pegged. The finished concrete pad shall have a brushed surface and a minimum of 2% slope in all directions from center to ensure proper drainage. While maintaining the road grade. All edges of the pad shall transition smoothly to the surrounding asphalt pavement not to exceed _-inch in height above the pavement.
