The work involves Firewall, Licenses, Core Switches, Layer Switches, and Installation. The successful vendor must possess a valid Universal Service Administrative Company (USAC) Service Provider Identification Number (SPIN), an active Service Provider Annual Certification Form (SPAC), and a Green Light Status with USAC at the time of the proposal opening in order to perform the work. The Contractor's SPIN, SPAC, and Green Light Status must be clearly stated in the Vendor's proposal. For public works components, the payment of prevailing wages is required. The successful vendor for public works components must also possess a valid and active Type C7 and/or C10 license at the time of the RFP opening. The Contractor's California State License number must be clearly stated in the vendor's proposal. Subcontractors must possess the appropriate license for any specialty subcontracted. Proposals for public works components must be accompanied by a bidder's bond, cashier's check, or certified check for ten percent (10%) of the amount of the proposal, made payable to the District, as a guarantee that the vendor will enter into a contract if awarded. No vendor may withdraw a proposal for ninety (90) calendar days after the RFP opening date. Proposals must be submitted using forms prepared by the District.
